- Self-inflicted Injury
- Client approached nursing staff with scratches on both forearms. No staff witnessed the self-harm event. Nurse cleaned client’s arms and covered the scratches after client’s shower. Observation level modified from Q15 to LOS constant. Client will process with her therapist.
- Licensing follow-up: Licensing observed client within LOS of staff during facility visit and verified current safety precautions.
