On 4/15/23, resident approached RN stating she needed some bandages because she “self-harmed in the shower.” Resident noted to have superficial cuts to her left forearm, per RN. When asked what she used, she reported that she used her fingernails. No first aid required. Resident placed on a self-harm precaution safety plan with constant line of sight as a result of this event.
