On 5/3/23, staff received a note from resident that stated that they were suicidal. C-SSRS screening performed by RN. Resident scored high risk. She was placed on a suicide precaution safety plan with constant line of sight and placed in paper scrubs for safety. Therapy was notified of screening results for follow up with resident the next day.
