While completing routine room checks, staff called out to resident who was in the bathroom. Resident was crying and wouldn’t produce an appropriate response when called for. Staff entered the room and observed the resident with clothing around her neck. Additional assistance was called and clothing was promptly removed from resident’s neck. No injuries were obtained. Resident was placed on suicide precautions with constant line of sight and given paper scrubs. Therapist will follow-up for individualization/duration of safety plan.
