Resident began banging head on wall, pulling her hair out, throwing any object she could find, and assaulted staff several times. Resident was placed in a physical hold to maintain her safety and the safety of the staff. Once she was able to regulate, have a calm demeanor, and contract for safety, she was transitioned out of hold. No injuries sustained. Resident to remain on self-harm and assault precautions.
