A complaint received alleges that a nurse is observing naked children while they are in the shower.

Complaints # AR00039471 and #AR00039478 were found not in compliance. A Corrective Action Plan was submitted and approved.

A resident was transported to the hospital for treatment.

A resident reported swallowing a metal shaving found in her food. The resident was taken for evaluation. All questionable food was thrown away, and the can opener will no longer be used.

During the admittance process, a resident hit the front door several times causing the lock to loosen and the door to open. The resident was able to run from the facility and into the road. Police were called. Officers located and returned the resident to the facility approximately 20 minutes later.

A resident reported being choked and grabbed on the neck by a staff member during a physical altercation. The complaint has been UNFOUNDED by Licensing.

A resident was transported for acute care after hitting a staff member four times.

A resident was transported for acute care after she became upset, punched a wall several times, attacked a nurse, self-harmed, destroyed property, and threatened others.

A resident reported that staff members held her down, put an elbow on her throat causing difficulty breathing, and bruised her arms during a restraint. This complaint has been UNFOUNDED by Licensing and Crimes Against Children Division (CACD). Staff will receive retraining.

A resident reported that a staff member threw her on the floor to restrain her, busting her lip. The resident also reported being bitten by ants in her bed and not being given the toothpaste she received at a dental appointment.

An annual review was conducted. All items were accounted for in hard copy files. No deficiencies were noted.

A resident began self-harming by biting her right and left hand. The facility was cited for late reporting.

Licensing visited the facility to conduct a Buildings and Grounds walkthrough. The facility was cited 911.6 for a broken light fixture in one bathroom and ceiling tiles out of place throughout the facility.

A resident was taken to the Emergency Room for dizziness, headache, and coughing up a dime-sized sputum of blood. After the evaluation, the resident was transported back to the facility.

A resident was transported to the Emergency Room for possible seizure-like activity. After the evaluation, the resident returned to the facility.

A resident walked into the nurse’s station, opened the medication cart, and took medication that did not belong to her. The resident was transported to the Emergency Room for evaluation. The facility was cited for 110.9.a, 110.17, 907.2, and 908.4 as a result of the incident.

While at the emergency room for a self-harm incident, a resident walked out of the ER and ran. Police located the resident and returned her to the facility.

A resident was sent to Ashley County Medical Center after injuring her hand by repeatedly punching a wall.

A resident’s family reported finding a lump on the resident’s head during a visit. The resident stated the injury occurred during a restraint, when he was pushed down and his head hit the floor. The resident also stated the staff member hurt him and twisted his arm behind his back.

Licensing performed a Buildings and Grounds walkthrough. The facility was cited 911.6 for issues identified during the visit.

A resident was transported to the emergency room after swallowing a dry wall screw.

A Buildings & Grounds follow up was conducted to inspect previous deficiencies.

No deficiencies were cited during the Inspection of Care (IOC) conducted on 09/23/24.

Children’s Records reviewed. All items present in hardcopy file except for daily/nightly observations, which were present in a separate file.

Complaint #AR00033801 was in compliance.

A resident was found in the bathroom with a pair of pajama pants wrapped around her neck.

A resident eloped during a trip to the Ankle and Foot Clinic in Little Rock. The resident returned to the facility on 7/26/2024.

A resident was transported to the emergency room after taking several medications she had been cheeking. The facility was cited for 907.2 as a result of this incident.

Officers responded to the facility in reference to a resident taking medications she had been pocketing. Officers assisted as Pro Med Ambulance Service evaluated and transported the resident to the hospital.

A resident was found in her bedroom attempting to pull a pair of pants around her neck.

A resident was found in the bathroom with a sheet wrapped around her neck.

Licensing conducted an after hours visit. No deficiencies cited.

A resident alleges a staff member poked her ring into the resident’s arm during a personal restraint, approximately a year ago. The resident alleges a second staff member twisted her arm during a restraint on 4/24/24. This complaint was UNFOUNDED by Licensing.

A resident was transported to the emergency room and then to acute care after cutting herself with metal from a paper towel dispenser.

A resident reported that her roommate got into the resident’s bed, attempted to kiss her, and pulled her shirt up to feel her breast. The complaint was UNFOUNDED by Licensing. The facility was cited 110.12 for late reporting.

Licensing visited the facility to conduct a buildings and grounds walkthrough. The facility was cited 911.6 for a loose toilet seat, torn wallpaper, and droopy ceiling tiles.

Licensing visited the facility to conduct an annual policies review and a personnel background checks review. No deficiencies cited.

Officers were dispatched to the facility in response to a juvenile assaulting a staff member.

Licensing visited the facility to conduct a buildings and grounds walkthrough. The facility was cited 911.6 and 912.4 as a result of the review.

During a video review, a staff member was observed grabbing a resident’s neck and slamming him to the floor during a restraint. The facility was cited 905.10 as a result of this incident.

Licensing visited the facility to review video for the early morning hours of 11/3/23 and 11/4/23. The facility was cited 907.6 for issues identified during the review.

Licensing visited the facility to conduct a buildings and grounds review. The facility was cited 911.6 for issues identified during the review.

Complaint #AR00030833 was not in compliance, all or in part, with deficiencies cited at N128 and N132. A revisit was conducted on 12/13/23, for all deficiencies cited on 10/11/23. All deficiencies have been corrected, and no new noncompliance was found.

Deficiencies were noted during the Inpatient Psychiatric Inspection of Care (IOC) conducted on 10/02/23.

Licensing and staff met to discuss the progress of the current Corrective Action Agreement made on 3/22/23. Due to recent citations involving the use of inappropriate behavior management by staff, the current CAA will be extended an additional 3 months. Two new requirements were made on the CAA. Staff have been retrained on their restraint hold procedures prior to today’s meeting.

A resident accused a staff member of hitting him during a physical hold. The staff member was terminated. The facility was cited 905.4.g and 907.2 as a result of this incident.

A resident reported that a staff member grabbed her by the arm, pulled her out of her chair, slammed her into two desks and then onto the ground. The staff member was terminated. The facility was cited 905.4.g and 905.10 as a result of this incident.

Three residents eloped from the facility by jumping a fence. The residents were later found and returned by police.

Officers were dispatched to the facility in response to 3 juveniles who escaped.

A resident was transported for acute care after he hit his wall numerous times, attempted to tie his sheet around his neck, and put his head in a sink of water. The facility was cited 110.17 for late reporting.

907.2 – Citations on 02/13/23 and 03/01/23 regarding staff being asleep and failing to supervise the residents.
907.5 – Citation on 02/13/23 regarding the overnight shift staff failing to remain awake and provide appropriate supervision of the residents.
907.6 – Citations on 02/13/23 and 03/01/23 regarding staff failing to conduct visual checks during sleeping hours on each child at least every 30 minutes.

Licensing visited the facility to discuss the initiation of a formal Corrective Action Agreement between the facility and DHS.

Licensing conducted a follow up visit to address night staff staying awake and doing bed checks per licensing standards. The facility was cited 907.2 and 907.6 for issues observed during the visit.

Two residents reported they had consensual sexual intercourse after one resident snuck into the other’s room. During the video review, two staff members were observed sleeping when the resident snuck into the peer’s room. The facility was cited 907.2, 907.5, and 907.6 as a result of this incident.
