Officers were contacted by Dell Children’s Hospital in Austin, TX where a juvenile was being treated. The juvenile disclosed that she was sexually assaulted or possibly raped sometime within the last year by a staff member while she was at Little Creek.

A resident was transported to the hospital for assessment after slipping in the shower and striking her head on the wall.

A resident was transported to the Emergency Room after accidently hitting his head on the corner of the TV while standing up.

Call for service states that CPS from Texas called to speak to an officer about a resident from the facility.

Four residents eloped from the facility through a bedroom window. The residents were located at a nearby gas station and returned to the facility by police officers. The facility was cited as a result of this incident.

Officers were dispatched to the facility in response to 4 runaway juveniles. The juveniles were located at a nearby gas station. Three of the juveniles were recovered and transported back to the facility. The fourth juvenile was located several hours later and transported back to the facility

A complaint received reported multiple issues at the facility.

A resident was assessed for a knee injury after dislocating her knee while sitting down on a couch.

A resident was sent to the hospital after presenting to the nurse as drowsy, weak, very pale, and complaining of “not feeling right.”

Officers were dispatched to a battery report call and made contact with complainant by phone. Complainant is staff member who reported that she was attacked by a resident at the facility.

Complainant advised that a coworker followed them home from work. The complainant believes this was due to an incident earlier in the workday.

A resident reported that he was pushed by a staff member. The resident reported that he was upset and tried to run off the unit, and the staff member pushed him back into the unit. When the resident fell into the unit, he tripped over a table and injured his left wrist.

An investigator with Crimes Against Children Division (CACD) contacted officers to report an allegation of sexual assault between two residents that occurred at the facility.

Officers were dispatched to the facility in response to a welfare check request. The reporter stated her daughter was posting on social media and should not have access to a phone.

A resident attempted to elope then began self-harming and reported suicidal ideations. After receiving a chemical restraint, the resident began exhibiting abnormal body movements and was sent to the hospital for evaluation.

Officers were notified by the facility of a resident who had made threats toward a school in Tennessee and a list of individuals. Officers reached out to the Sullivan County Sheriff’s Office in Tennessee to advise them of the situation.

A resident complained of pain to her hands after playing a game called hot hands/slapping hands with a staff member. The facility was cited 109.1.g for Unprofessional Conduct in the practice of child welfare activities.

An officer was dispatched in response to an allegation of sexual abuse of a juvenile.

A resident reported that her roommate makes her feel uncomfortable, has inappropriate boundaries, and has touched her inappropriately. The resident later recanted the statement. This complaint has been UNFOUNDED by Licensing and UNSUBSTANTIATED by Crimes Against Children Division (CACD).

A complaint was received requesting that Licensing review a report concerning an oversight gap related to the facility that has led to compliance issues that affect the safety and well-being of the children residing there.

Officers took a report of a sexual offense that occurred at the facility. The allegation states that two residents entered another resident’s room, pulled down his pants and underwear, and one of the residents inserted his finger through the resident’s underwear and into his anus.

A resident was transported to Baptist Health Hospital after reporting that she had ingested half of her 3oz facewash while in the shower and was feeling nauseous.

A juvenile reported that two other juveniles entered his bedroom and sexually assaulted him. The facility was cited 905.4.c and 907.2 as a result of this incident.

Officers were dispatched to complete a welfare check on a resident at the facility. The complainant stated that there was conflict between her and the facility, and she was not receiving answers related to her daughter’s welfare.

A resident reported her roommate touched her private area under her shirt while she was sleeping. The complaint was UNFOUNDED by Licensing.

Officer received a call for service from the Crimes Against Children division in reference to a sexual offense that occurred at Little Creek.

A complaint was received alleging that multiple residents were overmedicated. According to the complaint, weight gain, sleepiness, slurred speech, visible shaking and trembling were observed among the residents listed. The facility was cited 905.11, 905.12, and 908.8 as a result of this complaint.

Officer made contact with an investigator from the Arkansas State Police who reported a complaint of sexual assault that allegedly occurred at the facility.

Officer took a call for service regarding a sexual offense that occurred at Little Creek Behavioral Health. The Complainant with AR State Police Crimes Against Children informed officer that JV (Juvenile Victim) said that JO (Juvenile Offender) raped him from behind, sometime in mid-August.

A resident reported her peer forcibly placed her fingers into her vagina while in their bedroom. Both residents admitted to this incident occurring; however, the peer stated they were horseplaying.

A resident was injured during a restraint after becoming combative with staff.

A staff member reported that they overheard a unit conversation regarding a sexual assault that took place around a month ago between residents.

A complaint submitted to Licensing stated that the complainant received two separate emails containing personal information about another child at the Little Creek facility. The facility was cited 108.1.a as a result of this complaint.

The facility was informed of an outside complaint regarding a resident. The complaint stated that the resident was overly medicated, lying in her own vomit and unresponsive, and received no help from nursing or other staff members at Little Creek.

A resident was transported to the local Emergency Department due to a possible broken ankle. The resident received the injury while running in a circle around a table, being chased by two of his peers.

A resident wrapped a TV cord around his neck and began chewing on the portion that went into the wall. The resident stated he no longer wanted to be at Little Creek or be around his peers. The facility was cited 110.17 as a result of this incident.

A complaint of physical abuse was received by the facility. A resident reported that she was injured during a restraint, receiving bruises to her inner arm. The restraint packet completed after the restraint does not note any bruises or redness, however, the resident showed her peers and the floor supervisor a bruise. The complaint was UNFOUNDED by Licensing.

A resident reported that she received a bruise while horseplaying with a staff member. The resident stated she punched the staff member in the arm, and he punched her back. The facility was cited 109.1.g and 907.3 as a result of this incident.

Licensing visited the facility to review children’s files. All files reviewed complied with the minimum licensing standards.

Officers were dispatched regarding a suspicious person at the facility. The male, who is with the Department of Child and Family Services (DCFS), was at the facility to visit a resident. False or unfounded call.

Licensing completed an after visit of the facility. No deficiencies cited.

Two residents eloped from the facility after one resident stole a staff member’s car keys and her peer snatched a staff member’s badge off her shirt. The residents were located by the Sheriff and returned to the facility.

A resident was found unresponsive on the floor of the bathroom with cuts to both thighs.

Caller requested EMS assistance for an unresponsive juvenile.

Caller reported that two young ladies came to his door asking for a ride. Caller believes they came from Little Creek Behavioral.

A resident was sent to the emergency room after swallowing two batteries.

A resident reported injuring his wrist while horseplaying with a peer. The facility was cited 907.2 for allowing horseplay.

A complaint received by Licensing stated that the facility was allowing a supervisor to work with two felonies. The staff member was terminated on 4/2/2024.

An officer was dispatched to the facility in response to a report of a harrassing voicemail.

An officer took a report from the Arkansas State Police Crimes Against Children of an allegation that a juvenile was choked by a staff member at the facility.

Licensing visited the facility to conduct a staff record review. All files reviewed complied with the minimum licensing standards.

A resident reported a staff member had physically harmed him, bruising his upper lip and right knee during a physical hold. The resident was seen to be disoriented and have slurred speech from a new medication, when consent for the medication had not been given. The complaint was UNFOUNDED by Licensing.

A complaint received states that staff threw a resident on the ground when she attempted to run off the unit. The resident said that children at the facility have placed each other in the clothes dryers and that staff do not monitor the laundry room. This complaint was UNFOUNDED by Licensing.

A resident reported that she was choked by a staff member during a physical restraint. This complaint was UNFOUNDED by Licensing.

A report states that a juvenile who ran away from the facility was also reported missing in Texas.

Licensing received a report of a suicide attempt at the facility that was identified through record review for an unrelated incident.

A resident reported that a peer walked in on her in the shower and began to help her bathe. The resident stated the peer touched her inappropriately in the vaginal area and would not leave when told to do so. The facility was cited 907.2 as a result of this incident, and one staff member was terminated.

An officer took a report from Arkansas State Police in reference to a sexual assault involving two juveniles which occurred at the facility.

A resident complained that when a staff member lifted her off the ground, her shirt raised exposing her chest. The staff member continued to escort her. The resident could pull her shirt down and cover herself when on the unit. During video review of the incident, the staff member is observed dragging the resident on the floor during the escort. The facility was cited 109.1.g as a result of the incident.

A resident was found with a vape pen and claimed the pen was bought by a staff member who she also claimed to be dating. The complaint was UNFOUNDED by Licensing.

Licensing received a complaint stating a client had a medication error on 10/15/23 when she took another resident’s medication. The guardian requested that the resident be seen by a physician outside of the facility for follow up and the facility reportedly denied this request. The report stated the resident had felt “high” after taking the medications and the facility reported they monitored her vitals which remained within normal limits. Licensing provided the facility with technical assistance for 907.2.

A resident asked to contact the Child Abuse Hotline with neglect complaints. The resident stated staff are breaking confidentiality, the nursing department sits on their phones and does not help residents when in need, and a therapist told one of the residents to “kill herself.” Child Abuse Hotline was called, not accepted. The complaint was UNFOUNDED by Licensing.

Investigator from Arkansas State Police advised officer of a report of sexual activity involving a staff member and resident at Little Creek. Report: Staff member stated to resident “why is your butt so big”, then proceeded to grab her butt when the resident walked by him. Officer provided incident report number to the investigator.

A staff member failed to use a proper PRT single-man restraint to transition a resident to the floor. No injuries noted. The facility was cited 905.10 as a result of this incident.

A resident complained of chest pain during recreational therapy. The nurse noted the resident had an increased heart rate. The resident was sent to the hospital for further testing.

Licensing visited the facility to conduct a buildings and grounds walkthrough. No licensing concerns noted during visit.

A resident reported that a staff member asked her an inappropriate question regarding her body and then touched her on the butt. The complaint was UNFOUNDED by CACD and Licensing.

Licensing visited facility to review incident report logs, hotline reporting log, fire drill, and severe weather log. Licensing discussed standard 110.17 (what incidents are considered reportable to licensing) with facility staff. The facility explained the reports Licensing was reviewing were not part of the medical record and instead part of the internal document written directly by the staff involved. The final report is then written and stored separately. Licensing requested copies of these reports. Due to facility concerns about what is considered a suicide attempt vs a suicide gesture, Licensing Specialist will seek guidance from upper management and the Child Welfare Agency Review Board regarding suicide attempts.

A resident reported that a staff member showed the resident two videos of the staff member and his girlfriend having oral and sexual intercourse. The staff member was terminated. The facility was cited 109.1.g as a result of this incident.

Three residents eloped from the facility by snatching a staff member’s badge from around her neck and leaving through the side emergency exit. The residents were later returned to the facility by the Faulkner County Sheriff’s Department.

Officer was dispatched for 3 runaway juveniles. Staff stated 3 juveniles took off running to the woods and changed clothes. Officers located the juveniles and returned them to the facility.

Licensing visited the facility to conduct a buildings and grounds walkthrough. The facility was cited 907.3 for staff being out of ratio.

A resident reported that one staff member pushed her against the wall and pulled her hair, and another staff member choked her while attempting to take a spray bottle from her. The facility was cited 109.1.g, 907.2, and 910.1 as a result of this incident.

An officer was advised of a report of facility staff members showing inappropriate photos to juveniles.

A complaint received reported that facility staff showed pornographic material to multiple residents from staff’s phone. The facility was cited 110.9 and 109.1.g as a result of this incident.

A parent requested an officer respond to the facility for a welfare check. The parent stated that she had not heard from her daughter in over a week and wasn’t able to get anyone at the facility to answer her calls.

Residents eloped from the facility while leaving the gym. The residents were returned to the facility by the staff on 4/4/23.

Officers responded to facility for a suspicious person inside a vehicle on the property. Upon arrival, the individual stated she was a night shift supervisor and that 2 juveniles ran away, and they were trying to locate them.

Licensing determined the facility’s mandated reporter policy did not comply with A.C.A 12-18-402 nor the Minimum Licensing Standards. Technical assistance for 102.3.h and 110.9 was provided.

A resident was sent to the emergency room for evaluation of low oxygen levels due to her asthma and history of covid pneumonia.

A resident reported a hand injury from punching the molding on the corner of a wall. An x-ray was ordered.

Guardians of a juvenile requested a welfare check after the facility called to inform them the juvenile had been sexually assaulted.

A resident was taken to an orthopedic clinic for evaluation after shutting his hand in the unit door.
