- Self-inflicted Injury
- Client used fingernails to reopen an area on her cheek from a previous self-harm attempt. Minimal bleeding noted and the area was cleaned. Client will remain on self-harm precautions until safe behavior is observed.
- Licensing follow-up: Licensing reviewed documentation to confirm current self-harm safety plan is in place for client.
