- Client was triggered by a peer and began using her fingernail to reopen superficial abrasions that caused minimal bleeding. Nurse cleaned abrasions and processed with client. Client will continue self-harm precautions with constant LOS.
- Licensing follow-up: Licensing visited facility and observed client within LOS of staff in classroom. Classroom was in ratio.
