On 4/24/23 at approximately 2010, resident used a piece of plastic to rub off old scabbing on forearms. RN called to unit for support. RN processed with resident and resident gave up contraband. This event resulted in minor bleeding that was tended to by nursing. No further incident occurred. Resident has since been placed on a self-harm precaution safety plan with constant line of sight. Therapist will determine appropriate duration and individualization of safety plan.
