Resident took off one of her socks and tried to tie it around her neck. Staff intervened and took her socks away. Staff were assigned to stay with patient and therapist came to process with resident. Nurse practitioner will meet with resident to evaluate patient and potential need for med changes or further treatment planning. Corrective Actions: Taking away any material that could potentially be used for strangulation. Patient removed from unit until she can contract for safety. Patient to remain in paper scrubs with no additional clothing until she is safe from self-harm and Suicidal Behaviors for 72 hours.
