Resident began antagonizing multiple peers, demanding for them to hit her in multiple situations and residents had to be separated. One instance turned into an altercation and that resident was removed as well. After residents were separated, this resident ran into her room and tried to lock herself in. Staff caught the door and did not allow that to happen. Resident began scratching at her hands and banging her head against the wall. Resident was not able to be de-escalated and had to be put into a physical restraint for safety. During restraint, resident continued to try and bang her head on the floor which was prevented by staff. Resident received emergency medication while restrained and was able to eventually calm down enough to process and be released. Resident placed on self-harm precautions.
