Licensing Follow Up, Notice of Incident, Self-Harm

  • Licensing (Self-inflicted Injury)
    • Client was transported to Ortho Arkansas for a follow-up appointment concerning a hand sprain. An x-ray was conducted, and it was determined there is now a fracture present. Client told Ortho Arkansas personnel that she hit a wall with her hand on 10/3/22. A hard cast was placed on client’s hand.
    • Licensing follow-up: Client punched a wall and was taken to the ER which initially revealed no fracture. Follow-up appointment revealed a fracture and client’s hand was placed in a cast.

Licensing Follow Up, Notice of Incident, Suicide Attempt

  • Licensing (Suicide Attempt)
    • Client reported to staff that he swallowed rocks while outside. Client was taken to the ER where an x-ray was conducted. No rocks were located on the imaging tests. The facility has been unable to locate an acute facility for the client.
    • Licensing follow-up: X-ray revealed no rocks.

Upon arrival, officers were advised the juveniles had returned. Officers made contact with both juveniles.

Elopement, Licensing Follow Up, Notice of Incident

  • Licensing (Elopement)
    • At approximately 1815 hours, 2 clients breached the door of their dorm and eloped from the facility. Both clients were off campus for less than 5 minutes before staff were able to return them to Destiny House. Neither client sustained any injuries.
    • Licensing follow-up: Both clients returned to campus in less than 5 minutes.

Assault Third Degree

Upon arrival, officers observed a group of juveniles attempting to fight and staff attempting to separate them. Officers made contact with Juvenile 1 who stated she wanted to leave the facility and would kill herself if she had to stay. J1 stated she tried to cut her left wrist with her fingernails earlier, and officers observed the injuries. J1 stated that on Saturday (9/17/22), between 2000-2100 hours, a staff member forced her into a hygiene closet and attempted to touch her and told her how big her butt was. J1 stated when she refused, he struck her in the head and told her when she got out of there he was going to find her and take her to his house. Officers notified the Child Abuse Hotline. Staff advised officers that juvenile did not need to be transported due to facility treating suicidal subjects, but officers advised it was policy. MEMS transported J1 along with 3 other juveniles to hospital.

Officers made contact with staff in reference to a battery call. Staff member stated while assisting other staff restrain juvenile, juvenile purposely kicked her in the face. Staff advised her nose was bleeding afterwards, and officers observed redness around staff’s nose. Officer advised staff to seek warrants.

Licensing Complaint, Notice of Incident

  • Licensing (Complaint)
    • Complaint received stating a suicide attempt was made by a client. Client was sent to the ER via MEMS for swallowing a rubber ball.
    • Licensing follow-up: Facility visited on 10/19/22 and incident report was reviewed for 9/29/22. Client was given a sensory ball to assist with calming her down, and client swallowed the ball trying to self-harm. Client was sent to the ER. The facility did not report this incident to Licensing and was cited for 110.17.

  • Licensing (Self-inflicted Injury 1)
    • While a peer damaged a fire alarm causing police to respond, client became dysregulated and punched a wall. Client was assessed and taken to the ER. X-ray results: No fractures present. This is the second time in 2 days that client struck a wall with her hand.
    • Licensing follow-up: ER visit revealed no fractures.

  • Licensing (Elopement 1)
    • At approximately 1759 hours, 2 clients breached the door of their dorm and eloped from the facility. Both clients were gone for approximately 1 hour before staff located them and returned them to Destiny House. Neither client sustained any injuries.
    • Licensing follow-up: Facility visited on 9/30/22 and camera footage reviewed. Clients were seen breaching the exit door and running outside during an altercation with other peers. Staff/client ratio was 2:8.

Elopement, Licensing, Notice of Incident

  • Licensing (Elopement 2)
    • At approximately 2325 hours, clients eloped from the ER where they were being evaluated for acute placement. Both clients were gone for approximately 1 hour before being located by ER personnel. Neither client sustained any injuries. Client 1 was transported to an acute facility and Client 2 was in the custody of DHS awaiting placement to an acute facility.
    • Licensing: “No follow up needed. Clients remain at facility.”

Battery Third Degree, Criminal Mischief First Degree, Use of Force

Officers responded to a disturbance. Staff advised officers that the situation started when a fight occurred outside between Juveniles 1 & 2. J3 then began hitting J1 which required most staff to respond to a “code red”. A fight then started inside, and a few children started breaking things. Staff stated they saw J5 attempting to leave out of a back door, and kicked the door until it had broken. This allowed several children to leave out of the back door, but all were located and returned to the location. After this, J1 stated J2, J3, and J4 began assaulting him, and he sustained a contusion to his right forearm. Upon arrival, officers observed a large disturbance, and juvenile standing on a counter kicking glass. Another juvenile was breaking parts of wooden counter and tables. While officers attempted to detain a juvenile, he became combative, and pepper spray was deployed. MEMS was called to assess injuries. J1 was taken to ACH (no serious injury determined) and pepper sprayed juvenile was assessed with no injury noted.

Licensing Follow Up, Notice of Incident, Peer Altercation, Visit Compliance Report

  • Licensing (Peer Altercation 1)
    • Client was involved in a physical altercation with several peers, in which he was struck multiple times. Client complained of pain to his forearm and was taken to the ER. X-ray results: No fractures present. Client also complained of back pain while at the ER, and an x-ray of his thoracic spine showed no fractures.
    • Licensing follow-up: Facility visited on 9/30/22 and camera footage reviewed. Staff/client ratio was initially 2:7, then 3:7. Client was sitting when a peer threw a foot stool at him, which client defended with his arm. Staff intervened in approximately 1 second and stopped attack from continuing.

  • Licensing (Peer Altercation 2)
    • Client pushed peer, resulting in 2 peers physically attacking him. Staff intervened and separated them. Nurse noted multiple raised areas on client’s forehead and face, and client was transported to the ER. Diagnosis: Concussion without loss of consciousness and a black eye.
    • Licensing follow-up: Facility visited on 9/30/22 and camera footage reviewed. Staff/client ratio was 2:8. Peer was seen attacking client, and staff immediately intervened and eventually separated clients.

    • Client became dysregulated and punched the wall in her dorm room. Client complained of pain and was taken to Ortho Arkansas for an x-ray. X-ray results: No fractures present.
    • Licensing follow-up: X-ray yielded no fractures. Client remains at facility.

Complaint Visit, Visit Compliance Report

  • Licensing Specialist: Clayton DeBoer.
  • Facility visited from 6pm – 6:45pm. Census: 63. Destiny House, EMAC, and EMCC were observed and all buildings were in ratio.
  • TA – Staff informed Licensing that the graffiti in Destiny House would be removed.

Headache, Licensing Follow Up, Notice of Incident

  • Licensing (Headaches)
    • On 9/23/22, client told staff he was having headaches as a result of a previous injury. Client stated he hit his head on the sink during his stay at JDC and never received proper treatment for his injury. Client was sent to the ER on 9/24/22. A CT scan was conducted, and no abnormalities were found.
    • Licensing follow-up: Client remains at facility. Continue to monitor.

Officers made contact with staff who advised they are having issues with the phones. No further police actions taken.

Licensing, Notice of Incident, Peer Altercation

  • Licensing (Peer Altercation)
    • Client and peer were in a physical altercation on the basketball court. Both were separated and assessed by staff. Client complained of pain to his finger, and staff were told to ice and monitor his finger. Client later complained of pain in his hand, and client was sent to the ER. X-ray results: No fractures present.
    • Licensing: “No follow up needed.”

Licensing Follow Up, Notice of Incident, Suicide Attempt

  • Licensing (Suicide Attempt)
    • Client was in the bathroom and was being too quiet, so staff entered the bathroom to check on him (client was on LOS protocol). Staff observed client with a t-shirt wrapped around his neck. Staff removed the shirt and client was assessed. Redness noted to client’s neck. On 9/21/22, client was transferred to an acute hospital.
    • Licensing follow-up on 10/25/22: Facility states that client continues to be on LOS (line of sight).

Licensing Follow Up, Maltreatment, Notice of Incident

Dual (Maltreatment)

  • It was reported that 2 weeks ago, a staff member told client that he wanted to take her to his home and “make her have his babies.” He then attempted to force her into the “hygiene closet” to have sex with her. The reporter suggested that while trying to force her into the closet, the staff member “touched her butt.” The client resisted, and the staff member hit her on the back of the head. Staff member reportedly would continually make comments about “meeting outside to take her to his home.”
  • Licensing follow-up: Facility visited on 10/5/22 and Specialist interviewed 3 peers who resided with client where the alleged staff member worked. Peers were asked if any staff member had been inappropriate with them. All 3 peers stated no staff were inappropriate with them, and that staff were overall nice to them. CACD investigation was unsubstantiated.

Licensing Follow Up, Notice of Incident, Self-Harm

  • Licensing (Self-inflicted Injury)
    • Client reported to staff that she placed a disposable toothbrush inside her vagina for pleasure and to self-harm. Client was unable to remove the object. Client was transported to the ER and the toothbrush was removed from client’s vagina. Client has a follow-up appointment with a gynecologist on 10/17/22.
    • Licensing follow-up: Results of client’s visit to the ER. On 10/24/22, the facility reported that client was on “line of sight”.
