• Injury – Arm Broken in Restraint
  • Client was sent to ACH after an emergency safety intervention resulted in client injury; Client’s arm was broken. Staff member sent home until investigation is concluded.
  • Child Abuse Hotline was called and accepted. Based on video footage and reviewing the CPI Techniques program used by the agency, Specialist Breedlove determined that the proper intervention was not used in the situation with client and cited the agency under regulation 907.2.

• Injury
  • Alleged victim was recently discharged from facility and had a black eye, two bruises on either side of neck, and a bite mark on his back. AV was seen to have these injuries on 4.24.21 when he was discharged. Unknown how he got these injuries; Some boys were also mentioned to have peed on his clothes. The facility was said to have cockroaches. The AV’s therapist allegedly knew about the abuse and didn’t report it, simply moved the AV.
  • Child Abuse Hotline was called and accepted. No follow-up details.

• Sexual Activity and Lack of Supervision
  • Resident 2 had his penis exposed in the day area, when Resident 1 motioned him to come here” and appeared to open his mouth as his face went towards Resident 2s genitals. The pair later kissed. Staff suspended until retraining or termination is determined.
  • Child Abuse Hotline was called and accepted. Video footage was reviewed, and Specialist Breedlove determined there was inadequate supervision and cited the agency for that P907.2.

•  Elopement
  • Resident stated he was angry because he was lied to, then got up and ran to the fence and climbed on top of the roof, running from side to side. After notifying other staff, resident could not be located. He was located an hour later and escorted back to facility.

• Resident Suicide Attempt
  • When staff went to check on resident in bathroom, resident slid a suicide note under the door. Staff opened door and found resident on floor with elastic from her underwear around her neck. Staff intervened to get elastic off her neck. Video was reviewed; Specialist reported that video didn’t show anything negligent on behalf of staff. Staff checked on resident after required amount of time.

• Injury
  • Resident stated she didn’t feel well with a headache, then fainted by her bed. The nurse came and worked with resident. Resident fainted due to syncope; Hospital trying to determine the cause.

• Resident Suicide Attempt

• •  Peers informed staff resident was about to hurt herself. When staff went to the resident’s room, she was standing on her cubby and putting cloth around the door hinge. She then placed her head through the loop briefly before staff was able to get it away from her.

• Resident Self-Harm
  • Resident wrapped mask strings tied together around her neck.
  • Video reviewed; All cloth masks were replaced with paper masks.

• Resident evaluated at ER after punching wall several times and being restrained. No breaks or fractures were diagnosed.

• Resident Suicide Attempt
•  Regs out of compliance –
  • R907.2 – supervision based on period of time resident was in room without being checked on.

• Resident transported to ER for infected toe.
