Juvenile 1 advised Officer that Juvenile 2 was being very loud, and J1 was tired of hearing her complain, so J1 told J2 to “get a life”. J1 stated J2 approached her and grabbed her hair, punching her in the face, bit her arm, and kicked her. MEMS transported J1 to Children’s Hospital. Staff advised Officers both would be placed in separate rooms.

Staff advised officer Juvenile was acting out and was being violent, so she had to help hold Juvenile down so a sedative could be administered. Staff stated that while holding Juvenile down, Juvenile bit Staff on wrist. Staff refused medical; Officer advised warrants.

2 Staff members stated a Juvenile had caused a disturbance during which she punched and bit Staff 1 on the arm, and punched and kicked Staff 2 in the chin. Both Staff refused medical and no injuries were visible. Officer advised warrants.

Juvenile 1 advised she was randomly jumped on/attacked by Juvenile 2. Juvenile 1 stated she would like to press charges. Juvenile 2 stated she attacked Juvenile 1 because she was talking “mess” about her and “enough was enough”. The staff advised video surveillance should have captured the incident if need arises.

• IC reported S1 bent his hand back on Sunday morning prior to breakfast. The client advocate and I reviewed the video and were unable to corroborate this complaint. Advocate called it in to hotline and it was accepted.
• Interim Corrective Action – S1 placed on leave pending further investigation. Licensing still investigating.

• Resident sitting on washing machine. S1 yanked her off the washer resulting in the IC being injured, twisting her ankle and hitting the back of her head.
• Interim Corrective Action – S1 terminated. Supervisor written up for using inappropriate language. Facility questioned about other residents viewing the incident. Additional staff were written up for not following the protocol.
• Final Corrective Action – S1 terminated. QBHP present who did not assist or intervene was written up and placed on probation. Shift supervisor who was called for assistance who failed to intervene/assist was demoted and placed on probation.

Staff notified officers that she had found a resident in the restroom with a coat tied to her neck and the door knob; no visible injuries; resident taken to Children’s Hospital for evaluation.

Upon Officers arrival, there was a large fight going on and it took Officer and Staff members some time to get things under control before Officer was able to get back up. Multiple fights between Juveniles took place, and Juvenile 1 threatened to kill another Juvenile. Juvenile 1 was given a court date for terroristic threatening. Officers observed another Juvenile on ground with large gash in her arm, and requested MEMS. Officers determined Juvenile had caused her own injuries to her arm, as Staff stated she recently had surgery on it and she constantly removes the bandages and re-aggravates the wound.
