Location: Conway, AR                       Population Served: Non-Arkansas residents ages 6-21. Deaf and hard of hearing unit.

  • ER Visit (Self-inflicted Injury)
    • Client became upset after being bullied by a peer. Client began screaming and cussing, trashed room, and threw their belongings everywhere. Client punched a wall several times, causing swelling to hand. Client received an x-ray on 8/9/22: Salter II fracture distal fifth metacarpal with angulation vertex dorsal suggestion boxer fracture component. Client transported out on 8/10/22. No follow-up details.

Call Card, Missing Juvenile

Officer made contact with Alaska Juvenile Probation Office, in which staff stated missing juvenile was being held at Mat-su Juvenile Health Facility in Palmer, AK, and is in their custody (ran from facility on 6/5/22). Juvenile has warrants and will possibly have additional charges from incidents on morning of 7/30/22.

    • Client was admitted to hospital due to Enterocolitis.
    • Licensing follow-up: Email sent on 8/3/22 states that client returned to Little Creek from the hospital on 8/2/22 around 1614.

Buildings and Grounds, Monitor Visit, Visit Compliance Report

  • Licensing Specialist: Clayton DeBoer.
  • Buildings & Grounds conducted from 10am – 11am. Census: 51. Areas checked: Minnows, Guppies, cafeteria, and outdoor enclosed gym. MARs checked for 5 residents. No licensing concerns noted during visit.
  • On 7/12/22, Little Creek Risk Management received a report involving a client, who reported that a staff member hit them on the head. Licensing visited the facility on 7/18/22 from 10:30am – 12:00pm, and interviews were conducted. No definitive evidence to support violation of minimum licensing standards at this time.

  • ER Visit (Accidental Injury)
    • Client was outside during recreational time and jumped off a rock, landing on the metal landscape border. Client sustained laceration to foot and was transported to the hospital. Client received 6 stitches and returned to the facility.
    • Licensing visit on 7/18/22: Client remains at facility. Continued monitoring by nursing.

Maltreatment, Notice of Incident

  • Maltreatment
    • Client reported that staff member hit them in the head. Client was interviewed and assessed by the nursing department for injuries; None noted.
    • Child Abuse Hotline was called and accepted. DHS will take over the external investigation and send the findings. The alleged staff member was placed on administrative leave until further notice. Internal investigation: Camera review and interviews were conducted. The allegation of physical abuse will be unfounded. Upon information gathered, the facility found that the client fell to the ground and hit their head on the wall to avoid being placed into a restraint.
    • Licensing follow-up: Licensing visited facility on 7/18/22 and interviewed client. When asked to demonstrate what happened, the client stated that other kids were “coming after me and I fell in another kid’s room” and that staff member “hit me on the head”. No definitive evidence to support violation of minimum licensing standards at this time.

ER Visit, Notice of Incident

  • ER Visit (Muscle Cramps)
    • Client visited the hospital due to an EPS episode, however, was diagnosed with muscle cramps. At 8:00am, the client was shaking, couldn’t turn neck to the right, face drooped, and blood pressure was elevated. The client received an IM of Benadryl at the facility and was calm. The symptoms presented again around 11:30am, and the client was sent to the hospital.
    • Licensing follow-up on 7/5/22: Client remains at facility. Will continue to monitor.

  • ER Visit (Dizzy & Lightheaded)
    • Client was transported to hospital for dizziness and lightheadedness. Client received one bag of fluids at the hospital and returned to facility.
    • Licensing follow-up on 6/24/22:  Client received bag of fluids and returned to Little Creek.

Staff advised officers that 2 juveniles ran away from facility at approx. 1910 hours. Officers were unable to locate. On 6/13/22, J1 was located and transported back to facility custody.

Staff advised officers that 3 juveniles ran out of the back of the facility and into the woods at approx. 2338 hours. Officer located all juveniles at approx. 0429 and transported back to facility. Juveniles stated they left out the front of the building and never entered any woods.

  • ER Visit (Accidental Injury)
    • Staff entered client’s room to wake up client for dinner. Client woke up, lost his balance, fell forward, and hit his head on the nightstand. Nursing staff conducted Q15 checks until around 10pm when client was transported to ER. Nursing assessment: Client had an approx. 2-inch scrape on the left side of the head with hair missing and a small area that required cleaning due to a gash. Client also had a large golf ball lump in front of his head. No follow-up details.

Maltreatment, Notice of Incident

  • Maltreatment
    • Licensing received complaint reporting that a client stated a staff member “pops” kids in the mouth when they cuss and shoves/drags clients by the arm to make them sit down. Complainant stated the Tennessee Child Abuse Hotline was called, but the agency has not reported it.
    • Licensing follow-up: Licensing called the Child Abuse Hotline on 5/24/22 and it was accepted but screened out. Licensing visited facility on 5/25/22 and discussed the complaint with staff. Staff stated they have not reported it to the hotline. Due to the nature of the complaint and a previous Technical Assistance given to the agency on 8/18/21 regarding regulation R110.9, the agency shall provide re-education to all staff on mandated reporting and provide the Licensing Unit with a sign-in sheet showing all staff have been re-trained. Staff reported the client was questioned and stated there was no physical abuse from this staff member. Client also stated this incident reflected lack of training for trauma. An internal investigation was conducted with staff and clients which deemed maltreatment unfounded. Staff reported Trauma Informed Training is scheduled for 06/09/22 and Mandated Reporter Training will be held within the next month.
  • Facility cited for R110.9: The agency failed to report suspected child abuse to the Arkansas Child Abuse Hotline.

  • Elopement
    • Client opened the side yard fence to the basketball court and ran into the woods (the basketball court is under construction with an open panel to the parking lot). Due to client immediately not returning, the Sheriff’s Department was called.
    • Licensing follow-up on 5/11/22: Facility visited, and area of elopement viewed. The side fence client eloped from is missing a bolt that secures the latch to open and close gate. Risk Management ensured maintenance would install the bolt and weld it (on 5/11/22). No clients will be allowed in the area until the bolt is installed.

Upon officers arrival, staff advised juvenile had ran out of a door at the facility and exited the property through a gate that had been taken down due to construction. Officers attempted to locate juvenile but were unsuccessful. On 5/9/22, juvenile was located on a bus going into Austin, TX by Texas PD. CPS took custody of the juvenile, and officer was advised juvenile had charges from a neighboring agency and would most likely be held by that agency as well.

Officer was dispatched in reference to runaway juvenile, who left on foot and was located at provided address. Juvenile advised officer he ran away from facility because he didn’t feel safe due to there being a lot of fighting and it made him uncomfortable, as well as gave him flashbacks to memories of his parents. Juvenile also stated staff have made him feel unsafe by yelling at him and threatening to put him in restraints when he gets in trouble. Juvenile was returned to facility.

    • Client and peer were coming from the rec yard when client ran towards the fence and climbed over it, ignoring staff directives. Staff began to follow client and the Sheriff’s Department was called. Client was located a couple of blocks away (approx. 30 minutes later).

    • Licensing received complaint stating resident, whom may be in danger to himself or others, was being discharged from the facility without a discharge plan. No follow-up details.

    • R911.15.f – A coat with approx. 2-2.5 ft long string found on client’s belongings. The string was cut and removed. Staff are to conduct a thorough search of all client belongings.

Notice of Incident

  • Maltreatment
    • Staff member reported to the nursing department that she might have hit a resident at the facility. Client stated he was in a physical altercation with a peer when staff member intervened and separated them, telling client to keep his hands to himself. Client then physically assaulted the staff, and staff retaliated by physically attacking client. Client stated that staff apologized to him later that night, and he apologized as well.
    • Child Abuse Hotline was called and accepted. Staff member was terminated immediately. Specialist visited facility on 3/16/22 and reviewed video of incident. The video showed staff separated clients, then client slapped at torso of staff. Staff member then threw a flurry of punches at client, the last one hitting client in the face. Facility cited.
