  • Elopement (2)
    • Client eloped with 2 peers from his unit using a key fob they forcibly took from staff. Client was able to get a fourth resident, and all 4 residents eloped from the facility. Police were notified.
    • Licensing follow-up on 5/10/22: Camera footage review found that staff was out of ratio due to a staff member leaving her assigned area. Additionally, staff on the unit did not take proper actions when her key fob was taken by the residents, nor did she provide the level of supervision necessary to ensure the safety of the residents. The residents were found in Jackson, TN; They will not be allowed to return to the facility.

    • Client forcibly took a staff member’s badge/key fob from her hand. Client returned to his room and later exited the hall with his peers and eloped from the facility. Police were notified.

    • Client was in an altercation with his peer in which he sustained a forehead injury. Client was taken to the ER. No follow-up details.

    • Report received stating that client has been mixing and selling prescription drugs in the facility and it’s unknown how the child got ahold of the drugs (the drugs are not from his prescription).
    • Child Abuse Hotline was called and accepted. Licensing follow-up on 5/9/22: Specialist will debrief with the CACD investigator after interviews are conducted. Specialist requested that the client’s room and belongings be immediately searched for any medication.

    • Client injured his ankle while playing basketball and was taken to the ER via ambulance.
    • Licensing follow-up on 5/9/22: Specialist requested the x-ray results via email. Results showed no fracture or dislocation.

    • Client eloped from the facility after an altercation with his peer. Client kicked open doors while running through the facility and eventually left the premises. Staff kept the client in line of sight at all times. No follow-up details.

    • Child Abuse Hotline was called and accepted. Licensing follow-up: On 5/5/22, Specialist emailed the facility to determine the outcome of the camera review and the amount of time between the residents eloping and when staff became aware. On 5/10/22, Specialist reviewed the camera footage and discussed the incident with staff.

  • The agency has failed to maintain substantial compliance with Minimum Licensing Standards. Areas of concern are centered around Buildings/Grounds, Behavior Management, Ratio/Supervision, and Unprofessional Conduct.
  • The attached CAA reflects the excessive citations and lack of sustained compliance at Perimeter Behavioral of Forrest City. Because of this, youth are not receiving the required care as intended by the Minimum Licensing Requirements.

  • Licensing Specialist: Chelsea Vardell.
  • Facility visited; Census: 61. MARs reviewed for 2 SRU residents and 1 PRTF resident.
  • Regs out of compliance –
    • R911.6 – Light cover in bedroom 202 is missing, leaving the fluorescent light bulbs exposed. Station 1 quiet room had damage to the lower part of the right wall, causing shards of wood to be exposed. SRU bathroom shower stall appeared to have a dark matter in the ceiling of the entrance to the shower. The specialist was unable to determine if this was mold or dirt. Due date for all concerns: 4/29/22.

    • Licensing follow-up on 4/22/22: Specialist requested x-ray results via email.

    • Client reported that he was choked by staff member after he put his feet on the wall and was flickering the lights on and off. Staff member was placed on suspension pending the investigation results.
    • Child Abuse Hotline was called and accepted. Licensing follow-up: On 4/26/22, Licensing Specialists visited the facility and reviewed camera footage. As of 6/13/22, CACD reported that this case is still pending

    • Licensing follow-up: On 4/26/22, staff reviewed the camera footage with Licensing Specialist, which showed 2 staff with 9 residents. An incident occurred off the unit, in which the 2 staff members left leaving the residents unattended. Four residents then use a staff key card to elope. A staff member then enters and sits with the 5 residents, unaware of the elopement. The original staff member returns and monitors the 5 residents, also unaware. Staff were unaware of the elopement for 42 minutes until the local fire department called to report 1 resident at their fire station.

    • Client reported that a staff member grabbed him on the arm, threw him on the bed, and squeezed his arm. Staff was placed on suspension pending the investigation results.
    • Abuse Hotline was called and accepted. Licensing follow-up on 4/12/22: Specialist discussed the incident with staff and reviewed 1 hour of camera footage. Due to the footage being a 3-hour window, it was decided that staff will continue to view the footage and alert Licensing to any findings. Licensing Specialist will return for further camera review if it is determined that staff did enter the resident’s room.

    • Client reported he injured his back 2 weeks prior when he fell off the stairs of his bed. Client was taken to the ER. No follow-up details.

  • Maltreatment (1)
    • Client reported he was pulled from his bed by staff and dragged down the hall to the nurses station. A camera review was conducted of the incident.
    • Child Abuse Hotline was called and accepted, substantiated. Licensing follow-up: On 4/8/22, staff reported they will be terminating the employee. On 4/12/22, Licensing reviewed the camera footage which showed staff dragging the resident from the room after he hid from staff. The resident was not a harm to himself or others at the time of the “restraint”. Additional staff witnessed the unprofessional conduct and did not report it to management. Staff witnesses were retrained by management on mandated reporting and received written disciplinary action.

    • Two clients reported that a staff member pinched them for talking during quiet time and client 1 disclosed a bruise on his left arm that was large and healing. Client 2 stated staff member would not let them get a grievance or therapist sheet. Staff member was suspended pending the outcome of the investigation.
    • Child Abuse Hotline was called and accepted, unsubstantiated. Licensing follow-up on 4/13/22: Staff reported one of the clients did not have any marks, and one had marks because of self-harm and was on restriction because of self-harm and fleeing. There is no video of the incident due to the facility getting new cameras. There was no documentation by nurse of bruises from being pinched, and staff denied allegations against him. The investigation was closed out unsubstantiated.
