Location: Springdale, AR                   Population Served: Adolescent girls ages 7-17

# of residents per unit: Up to 12 Residents per room: 2 Capacity: 32

Contact with family (Calls and visit schedule): Weekly calls up to 3x/week for 10 minutes, intervention call available 1x/week for 10 minutes. Visitation available 2x/week for 30 minutes if previously scheduled. Zoom available if in person is not possible.

Caller requested a welfare check on Juvenile at facility; Caller believes they are verbally abusive towards her. Caller advised that child services has custody and they told her to call police. Facility stated Caller (Mother) had been calling them all weekend and that Caller doesn’t have custody. Officers did not respond since Caller didn’t have custody.

Officer requested to meet at car in front of Children’s Hospital to speak with Caller in reference to a Juvenile being assaulted by one of the nurses at facility. Caller advised this occurred around 16:30 (calling at 21:12). Upon officers arrival, car was not occupied.

    • R913.5 – Child’s bedroom needs to contain bed, mattress, and bedding. There also needs to be an area to store personal belongings (Correction date 10/21/19).
    • R911.6 – Peeling paint needs to be addressed on doors, hallways, and resident bedrooms/bathrooms. Per Director painting will begin no later than 10/28/19, a work order was already in place to address this. Trim around sink in room 201 needs repaired, sink plugged in room 202, 203, toilet seat missing in room 303, shower tile needs repaired (Corrected on 10/22/19 for all). Toilet stopped up in room 305 and wet towels need removed (Corrected on 10/21/19). Broken switch in cafeteria needs repaired, toothpaste on walls needs to be cleaned and removed from walls, in bathrooms 205 and 204 clothes and trash need removed, small hole in education hallways needs repaired, missing linoleum or tile in classroom needs repaired.

  • Reviewed 9 children’s records and conducted partial walkthrough of building.

    • R912.3 – Hot water nozzle not working in one of the bathrooms.
    • R902.3 – Proof of immunizations needed for 4 residents.
    • R907.3 – Ratio was 1:9 in the classroom due to staff stepping out (Correction date 10/1/19).
    • R902.13 – Proof of legal documents or a request for the documents for 2 residents.

Caller requested a welfare check on Juvenile at facility; Caller has not heard from Juvenile and is getting worried (had been 36 hours). Caller stated when she spoke to Juvenile last, they were being threatened. Officers responded; Juvenile had been released to Mother on 25th. Officers unable to contact Caller.

Caller requested officer to facility to make contact with Staff in reference to 2 Juvenile runaways that have been located in Rogers. Staff stated they would need to go through their procedures to see if they can go get them or not. Transport approved, Officers picked up 2 Juvenile runaways.

Staff advised of several individuals parked near the facility with a dog that appeared to be incoherent and slurring their words. Staff does not feel comfortable leaving. Officers responded.

Staff advised there was an individual pushing a stroller that appeared to be “tweaking”, walking near the facility; Officer unable to locate.

  • Reviewed 10 personnel records, all areas accessible to residents, and medication logs.
  • Regs out of compliance –
    • R105.15.g – A copy of job description needed for 1 staff member (Compliance date 6/15/19).
    • R911.6 – Possible mildew/mold substance on bathroom ceiling should be checked in room 301. Housekeeping should be addressed in bathroom and homemade bathroom light covers are blocking almost all the light in room 303. Bathroom light needs repaired, and shower tiles need replaced on shower tile floor in room 304 (Compliance date 6/15/19).
    • R105.15.h – Additional non-relative reference needed for 1 staff member (Compliance date 6/15/19).
