Location: Springdale, AR                   Population Served: Adolescent girls ages 7-17

# of residents per unit: Up to 12 Residents per room: 2 Capacity: 32

Contact with family (Calls and visit schedule): Weekly calls up to 3x/week for 10 minutes, intervention call available 1x/week for 10 minutes. Visitation available 2x/week for 30 minutes if previously scheduled. Zoom available if in person is not possible.

Licensing Compliance Record

  • R405.5 – Complaint of staff using inappropriate restraint on child is founded. Agency responded  appropriately and terminated the offending staff member.
  • Resident reported via grievance that staff member body slammed her on 01/08/21. Video Review confirmed. AO was previously suspended on 5/18/20 for telling a resident “I hope you break your neck.” He was put on 90-day probation then and successfully completed it.
  • Interim Corrective Action: AO terminated “due to body slamming of AV because this was inappropriate handling of child and was not SAMA…”
  • Accepted by hotline.

Caller advised someone came into facility and left a very expensive radio there, and would like someone to pick it up. Officer picked up radio from facility.

Call was short, not enough time to determine if call was an emergency. Staff spoke with Officer stated there was no emergency, the call was made by a Resident that thought it was funny.

Caller is wanting to report a neglect for Juvenile at facility; Caller is in Ohio and wants a full investigation about this stating that a welfare check would not be enough. From what Juvenile described, everything was consensual; There will be no report. The Mother was given her options in reference to having Juvenile possibly moved from the facility.

Caller reported van parked in front of the facility and had been running for some time. Had been parked there for almost 5 hours.

Caller (parent, adopted Juvenile) advised she was calling on behalf of Juvenile at facility; Caller stated Juvenile had a No Contact Order against grandfather (biological) but he had been sending letters and packages to Juvenile in facility. Officer discovered there was no active No Contact Order against grandfather. Staff advised that they talked to Caller and were holding a letter from the grandfather to give to parent as grandfather was not on the preapproved individuals list from parents. Caller wanted to press charges; Officer advised Caller of her options.

  • Licensing complaint received 2/2020 is founded for inadequate supervision.
  • Corrective Action: Re-training of the employee.
  • Employee was terminated from facility after investigation.

Staff advised there was an individual going around the building and trying to get into the facility, pushing everyones doorbells. Individual was stating she had just moved there and was out for her first walk and came to see what they were. Officers responded.

Caller requested a welfare check on Juvenile at facility; Caller believes they are verbally abusive towards her. Caller advised that child services has custody and they told her to call police. Facility stated Caller (Mother) had been calling them all weekend and that Caller doesn’t have custody. Officers did not respond since Caller didn’t have custody.

Caller advised there was a juvenile walking up the off ramp and looked either very cold or crying. Officer responded.

Officer requested to meet at car in front of Children’s Hospital to speak with Caller in reference to a Juvenile being assaulted by one of the nurses at facility. Caller advised this occurred around 16:30 (calling at 21:12). Upon officers arrival, car was not occupied.

Visit Compliance Report

  • Regs out of compliance –
    • R913.5 – Child’s bedroom needs to contain bed, mattress, and bedding. There also needs to be an area to store personal belongings (Correction date 10/21/19).
    • R911.6 – Peeling paint needs to be addressed on doors, hallways, and resident bedrooms/bathrooms. Per Director painting will begin no later than 10/28/19, a work order was already in place to address this. Trim around sink in room 201 needs repaired, sink plugged in room 202, 203, toilet seat missing in room 303, shower tile needs repaired (Corrected on 10/22/19 for all). Toilet stopped up in room 305 and wet towels need removed (Corrected on 10/21/19). Broken switch in cafeteria needs repaired, toothpaste on walls needs to be cleaned and removed from walls, in bathrooms 205 and 204 clothes and trash need removed, small hole in education hallways needs repaired, missing linoleum or tile in classroom needs repaired.
