At 1735 hours, caller advised juvenile ran away from facility. At 1748 hours, caller advised the juvenile was at 2710 S 48th with a nurse; Officer responded to location and juvenile was transported to NWMC for suicidal ideations. Juvenile was returned to facility at approx. 1930 hours.

On 5/10/23 at 20:00, patient self-harmed by scratching self with a piece of plastic on forearms. Staff processed with patient. Contraband was retrieved by staff.
