- R1012.4 – Room 204 had visible debris and an unknown substance splattered on the toilet. The restroom in the dayroom smelled of urine. Corrected 12/20/21.
- Regs not correctable –
- R1008.3 – Medication not initialed as administered for 3 residents; Nurse is unsure about 2 the previous day but stated for 1 resident that she had no time due to being the only nurse on the unit having to handle other resident situations during administration. Specialist informed her they must be initialed during administration.

- R1002.13 – Multiple residents have no reason for admission documented (documented the following day); Ensure that moving forward, all information required is documented at time of admission.
