- Staff placed on suspension pending investigation as of 9/25/21. Child Abuse Hotline was called and accepted, unsubstantiated. Specialist sat in on interviews with DCFS investigator at facility and reviewed copies of the nurse’s notes regarding the injuries that resident sustained. Six residents were interviewed (witnesses) and all reported that they did not see any kick from the staff member to the resident.

Staff reported 3 juveniles were on the roof and 1 stated he would commit suicide. Staff didn’t know how the juveniles got on the roof. Juveniles were walking around on the roof, sitting on edges, and laughing at staff. Juveniles got off the roof while police were on scene; No action requested by staff.

- Client accidentally got her right 5th finger caught in a door. As a result, client had a nail bed avulsion with moderate bleeding and no bruising. Client was transported to the ER. No follow-up details.

- No site visit conducted. Specialist received notification of an incident on 4/6/22 that occurred on 4/1/22. Incident involved resident reporting pain to his right hand to nurse on 4/1/22 due to punching a wall approximately a month prior. An x-ray was ordered and showed a subacute fracture to the fifth metacarpal bone. The resident will follow up with Orthopedic care. Specialist followed up with staff on the cause of the reporting delay: “The nurse forgot to inform the DON that the x-ray occurred – as soon as we received the results the x-ray was forwarded to me and Angie. However, we did provide prompt medical care and he has a follow up on Tuesday at 1300 at Ozark Orthopedics.”

- Client was assaulted by 2 peers in which he was hit and bit. Police department was notified and client was transported to the ER.
- Specialist contacted facility on 3/30 to determine if the residents have been placed on restrictions from one another and if the residents who assaulted client were charged with any violation from the police.

Upon officers arrival, staff stated a large fight broke out between the juveniles at the facility. Staff member stated she was hit with a chair. A juvenile had injuries to his face and arms. Staff asked officers to leave due to carrying firearms. Another staff member met officers outside, stating that while she assisted in holding a juvenile back from the fight, the juvenile grabbed her by her thumb and bent it backwards to her arm and then pushed her into a wall and kicked her in the stomach. Staff member stated facility has no safeguards for their employees and they are unable to defend themselves when being attacked. She did not want to pursue charges and stated her employer would do nothing to help her. That staff member was transported to the hospital.

- Client eloped while transitioning back to the unit from outside. Client went through the double gates leading to staff parking lot. Local law enforcement was contacted and staff maintained visual of client as he went across the street to a gas station. Client stole a snack and pushed staff to the floor.
- Specialist followed up on 3/30: Facility reported that staff had left the gate unlocked. Facility reported that both department heads of the employees who use this gate were informed to remind staff to ensure the gate remains locked at all times.

Caller advised a juvenile left the facility on foot. Upon officers arrival, juvenile was with staff and was given a ride back to the facility. Juvenile was cooperative.

- Client eloped from the gym after forcing open a door and ran across the street off facility grounds. Staff followed client across the street and was able to process with him. No follow-up details.

Client sustained abrasions to the right ring and middle finger and complete nailbed injury to the right index finger. Minimal bleeding noted, and some slight bruising noted to all 3 fingers. - Client and peer were in altercation in which peer hit client in the head. Due to a pre-existing diagnosis, client was taken to the ER. Client was then transferred to Mercy Hospital in Springfield, MO as a precautionary measure.
- Specialist emailed facility on 3/15 to inquire about peer restriction. Staff reported the residents cannot be moved to different units due to their age. Staff reported the residents are on a 6-foot rule, but they are unsure how long as it is decided day to day.

Two juveniles from Piney Ridge got onto Vantage Point roof and threw ice/HVAC items down to discourage staff from coming up. They then climbed down to runaway on foot, but staff eventually caught up to them and they returned to facility.
