Location: Fayetteville, AR                   Population Served: Children and adolescents ages 7-17 with sexually maladaptive behaviors and other co-occurring psychiatric conditions.

Visit Compliance Report

  • Unannounced monitoring visit; Partially viewed building and grounds, viewed medication room and logs.

    • R1011.7 – Hole in classroom in admin building (created on 10/10/19). One bathroom each on East & South units have urine odor (agency using several different chemicals to combat this odor). Wall in bathroom 201 needs to be cleaned, busted pipe in day room bathroom on East unit (work order in place, waiting on new part order to be delivered). (Correction date 11/26/19)
  • TA –
    • The agency has been repainting several areas in the facility this week (several areas have already received scratching within 24-48 hours of painting). Windows that needs replaced (some have been replaced and some are still on order). Agency plans to keep additional windows on hand to avoid six-week delay in the future. Agency trying several new chemicals for bathroom cleaning and looking into putting epoxy flooring/wall in all bathrooms.

Complaint Survey with POC, Revisit Survey

  • Complaint #AR23618 substantiated with deficiencies cited at:
    • N 131 Protection of Residents – Failed to ensure a chemical restraint and seclusion were not used simultaneously used to assure the safety of 9 residents.
    • N 209 Facility Reporting – Failed to ensure a serious occurrence report was maintained in the clinical record for easy reference for 2 residents.
  • Plan of Correction – Director of Nursing reviewed records, in-serviced nurses, and updated the ESI policy and checklists. Director of Nursing/Administrative Assistant will monitor to ensure compliance.
  • 11.26.19 Revisit for 10.11.19 deficiencies. All deficiencies have been corrected, and no new noncompliance was found.

  • Incident on 8/18/19: Interim Corrective Action Plan no longer required. There is no evidence to support any licensing concerns, and the agency has a procedure in place regarding heightened supervision during hygiene time. The ratio at the time of the alleged incident was 5:26.

Misc Theft

Roadside Assistance was called due to an individual needing specialty nuts removed from flat tire. It’s suspected the individual with the flat tire stole the specialty lug key from Driver and attempted to extort him by filming him and going against the instruction to not drive a far distance since Driver had to tighten the new tire with a pair of plyers.

Caller reported 2 children were physically fighting in the parking lot. Upon arrival, Officer reported no fight occured, just a child not obeying parent.

Caller reported an individual pulled up beside her in their car and when opening the driver door, hit the passenger door of Callers car. After asking individual if there was any damage, individual said no. Upon Callers inspection, several small scratches were on the passenger door. Officer determined only 1 scratch would have been possible based on the location of the door.

Caller reported runaway juvenile and that a staff member might have located the juvenile but isn’t sure. Officer no longer needed, staff advised the juvenile had been located and was being transported back to facility.

Welfare Concern

Caller requested a welfare check because of a letter they had received from resident at facility. Caller reported in the letter the resident stated they were getting beat up by another resident. After Officer made contact with resident, they stated they wrote the letter because other resident was being mean; Resident stated they were not friends. Resident was in good health and stated everything was fine.
