Visit Compliance Report

  • Specialist and Assistant Director conducted unannounced monitoring visit, viewing all areas of facility. After completion of walkthrough, debriefing was held with CEO. Several residents were not wearing face masks during this time of pandemic.
  • Regs out of compliance –
    • R912.6 – There was not any soap in the soap dispensers in the community restrooms (Correction date 8/17/20).
    • R912.4 – Wet tissue on floors, dirty laundry on floors on East unit rooms 305 and 306 and North unit rooms 204 and 206 (Correction date 8/17/20).
    • R911.6 – Trash on floors, beds not made, wet tissue and dirty laundry on floors in East unit rooms 305 and 306, and North unit rooms 204 and 206 (Correction date 8/17/20).

    • 912.4, 1012.4, (West Unit) – Dayroom bathroom smells of urine. Water damage to wall behind toilet.
    • 912.4, 1012.4, 911.6, 1011.7 (East Unit) – Dayroom bathroom has urine smell. Hallway had large hole in wall.
    • 912.4, 1012.4 (Gym) – Bathroom sink clogged. Urine smell in bathroom.
  • TA – Discussed agency is in process of painting units and installing epoxy in bathrooms.
