Notice of Incident, Peer Altercation

  • Peer Altercation
    • Peer appeared to have purposefully stepped on client’s right hand after client crumpled up peer’s paper airplane that landed near him. Nurse noted finger to be slightly red and swollen. After assessment the next day, an x-ray was ordered and returned normal.
    • Licensing follow-up: The residents were separated and are on different units. No licensing concerns noted.

  • Accidental Injury
    • Client was playing in football tournament when he fell and landed on his right wrist. An x-ray was completed in-house with no abnormalities noted. No further treatment required at this time.
    • Licensing follow-up: As of 12/06/22, Licensing has not been informed of any further complaints from client. No licensing concerns noted.

Notice of Incident, Self-Injury

  • Self-inflicted Injury
    • Client became upset and hit a wall with his right hand. An x-ray was completed in-house and showed a fracture of the fifth metacarpal. Client will follow-up with orthopedic clinic on 11/23/22.
    • Licensing follow-up: On 11/23/22, client was placed in a soft cast for the 4th and 5th metacarpals acute fracture. Client has a follow-up appointment on 12/02/22. No licensing concerns noted.

  • Accidental Injury
    • While playing basketball, client’s hand struck the ball causing injury to the small finger on his right hand. Swelling with pain noted to finger. X-ray was ordered and was negative for a fracture or dislocation.
    • Licensing follow-up: Facility reports client has not reported any further pain to the nurses. No licensing concerns noted.

Notice of Incident, Pain after Restraint

  • Pain after Restraint
    • Client complained of left shoulder pain after a restraint. An x-ray was completed in-house and showed no fractures. As of 11/21/22, client has no pain or complaints in that shoulder area. No follow-up is scheduled at this time.
    • Licensing follow-up: Facility reported there is no video footage of the restraint due to the camera pointing down at the floor. Staff interviewed the resident, and staff were questioned on the incident. It was reported that the staff member attempted to place the client in a Handle with Care one person hold with the resident struggling against the hold and the staff member needing to reposition the restraint. On 12/12/22, the CEO informed Licensing that no further pain was reported by the client. No licensing concerns noted.

Welfare Concern

Caller requested a welfare check on juvenile; Caller is investigating an assault that juvenile is a victim of. Officer spoke with juvenile who was fine and had no complaints of injuries.

  • Accidental Injury
    • Client complained of pain to his ankle, and an x-ray revealed a possible Salter Type 1 fracture. Client went to Ozark Orthopedics on 11/14/22 and received diagnosis. Client was provided with a walking boot and ordered to have a follow-up in 2 weeks. Client reported that last year, he twisted his ankle in school. It is unknown when the healing fracture occurred.
    • Licensing follow-up: On 12/06/22, Licensing was informed that the client was no longer in the walking boot. No licensing concerns noted.

    • While playing basketball, the ball hit client’s finger and caused disfiguration. Client was taken to the ER.

    • Client was participating in recreational therapy and fell on his left arm. Client reported pain when he tried to move his arm. First aid was provided, and an in-house x-ray showed no fracture.

  • Licensing (Accidental Injury)
    • While playing basketball, client’s finger was hit after he tried to catch the ball. Client’s finger began to swell and turn a purple color, and an x-ray was ordered. Upon receiving the x-ray results, a follow-up appointment was made at Ozark Orthopedics. Client was placed on no sports.
    • Licensing follow-up: Specialist was informed that client was seen at Ozark Orthopedics on 10/25/22, and an additional follow-up is scheduled. No licensing concerns noted.

  • Licensing (Self-inflicted Injury)
    • Client became upset and hit a window multiple times. After client was able to calm down, he complained of pain and swelling in his hand. An ice pack and Ibuprofen were provided, and an x-ray ordered. X-ray results were received on 10/24/22. Facility will continue to monitor his symptoms and order follow-up if needed.

  • Licensing (Peer Altercation)
    • Client and peer were in an altercation on 10/15/22, during which client fell and landed on his elbow. At the time of the injury, client reported minor pain in his elbow and no bruising or swelling was noted. On 10/19/22, an in-house x-ray was conducted after client complained of pain, and the x-ray showed no fractures. On 10/20/22, client complained of increased pain in his elbow and was transported to Ozark Orthopedics for an x-ray. Client was placed on no sports.
    • Licensing follow-up: Specialist followed up with facility on 10/31/22 and 11/4/22 regarding client’s appointment with Ozark Orthopedics. Facility advised the client has a follow-up appointment scheduled. No licensing concerns noted.

Suicide Threat

Caller who brought juvenile to facility stated juvenile was refusing to get out of the vehicle. Juvenile threatened to kill siblings with a knife earlier in the day in Fort Smith. Juvenile was detained and transported to children’s hospital via CEMS.

  • Licensing (Peer Altercation)
    • Client and peer were arguing outside and peer threw hard dirt at client, hitting him in the eye. Client denied any pain or difficulty seeing at the time, and first aid was provided (washing eye and applying ice). Client was seen by an eye doctor, and client continues to deny any pain or difficulty seeing. No follow-up was scheduled.

    • Client tied a sweater around her neck while in the bathroom. After speaking with staff, client gave the sweater to staff. No injuries were noted to client’s neck. Client was placed on suicide precautions.

Police Report

Noise Complaint

Caller (resident at apartment complex nearby) stated loud music was coming from facility area and was playing “all day yesterday and started up again at noon”. Officers responded, and no excessive noise heard in area. Officer advised caller to call back if noise continues.

    • Client’s nose began to bleed while in the cafeteria, and client was taken to the ER due to client’s history of needing a cauterization procedure in the past due to nosebleeds. Client returned from the ER with new orders for a nose spray (no procedures were completed at ER).
    • Licensing follow-up: No licensing concerns noted.

  • Licensing (Suicide Attempt)
    • Client was observed in the bathroom with a bed sheet tied loosely around neck. Client was talking with staff while nurses were able to remove the sheet and process with the client. Client was placed on suicide precautions which includes leaving the door cracked, 6 ft with staff at all times, and sleeping on a cot in the hallway.
    • Licensing follow-up: No licensing concerns noted.

    • Client reported he was having trouble breathing. Client was assessed and the nurse noted client’s vitals were out of range and he had swelling to both hands and his face. Client has a rash that he is currently being treated for with steroids. Client was transported to the ER, and later returned with a new order. Client denied any trouble breathing and the rash was starting to improve.
