On 12/1/24, a resident reported to her mother that she punched a wall with her right hand. The resident’s mother emailed the therapist about the incident. An x-ray was ordered.

A resident alleged that a staff member slapped him in the face. The complaint was UNFOUNDED by Licensing.

A maltreatment report received states that a staff member put his hands around a resident’s neck and performed an improper hold by holding the resident down with his arms and knee in the resident’s chest. A Medicaid Inspection of Care review was conducted at the facility in response to a complaint received.

A report received states that the facility currently has no teachers on staff as of 9/6/2024. The facility is reportedly using substitutes to make up for the lack of teachers.

Reported to AR Child Abuse Hotline: A resident had a piece of paper and refused to give it to staff, and the staff member grabbed her arm and tried to remove the paper from her hand, scratching the resident with her nails. The complaint was UNFOUNDED by Licensing.

A resident was injured while going down a zip line. The resident was transported to Arkansas Children’s Hospital for evaluation.

Client was transported to Methodist Behavioral Health due to her ongoing aggressive behavior, and while there she accused staff of choking her. This complaint was UNFOUNDED by Licensing.

During an incident with an aggressive resident, a staff member was observed grabbing the resident’s sweatshirt and pushing her to the ground.

An officer made contact with Cynthia Ragland at the 12th street patrol substation. Ragland reported that she was attacked by a juvenile at the Methodist Children’s home where she is a behavioral instructor.

A resident was transported to Arkansas Children’s Hospital after ingesting an unknown amount of Ibuprofen she had taken from the nurse’s station.

An officer transported a juvenile from the facility to Pinnacle Pointe Hospital.

Officers responded to the facility for a disturbance involving several juvenile males that were destroying property.

A resident reported she had been joking around with a staff member, but that led to the staff member putting the resident’s hand in a toilet. The resident also reported she ended up on the floor with a split lip at the end of the incident.

Upon arrival, officers made contact with a staff member who reported that a juvenile resident pushed him into a wall and began poking himself with a pen. When the staff member attempted to take the pen away, the juvenile scraped the staff member’s arm and stomach with the pen.

Licensing received a complaint on 11/1/2023 regarding a child who was picked up from the facility on 10/28/2023 by her guardian for a visit. The child was complaining of ongoing pain in her stomach, so the guardian took her to the emergency room and was then transported to ACH for further treatment of pancreatitis, a blood clot in her spleen, and sepsis. The child reported she was complaining to the nurses about the pain since Wednesday 10/25/2023 and they gave her ibuprofen on Thursday 10/26/2023. The child reported that when she continued to complain of pain, the staff told her to quit faking it. The complaint was UNFOUNDED by Licensing. and UNSUBSTANTIATED by CACD.

A complaint received states: It was reported to me that my son and one other resident tested positive for THC while in the care of United Methodist Children’s Home. Due to him testing positive for THC, he lost privileges to the outing which led to an aggressive outburst. I am concerned about how drugs entered the facility and the safety of the residents. Since this incident, my son is being discharged within the next 5 to 7 days.

A resident reported to staff that 2 peers had a cannabis pin inside of the building. The resident also reported that a staff member gave the residents the marijuana pin. Both residents were drug tested. Resident 1 tested positive for marijuana and Resident 2 tested negative. When asked, Resident 2 confirmed Resident 1 got the marijuana pin from staff. The staff member was informed of the allegations and stated she would never do anything like that. The staff member was sent to conduct a drug test at AEL and was suspended pending the investigation/drug screen results. Facility cited 109.1.g: Resident tested negative for marijuana upon admission to UMCH 8/23/23 and tested positive for marijuana 10/10/23. Resident has not been on any gate passes since admission. Resident was given access to marijuana whilst at UMCH. There is no definitive evidence at this time to support specified staff provided the marijuana, but it is evident resident had access to marijuana whilst at this facility.

Staff advised officer he wanted to make an assault report. Staff stated that on 10/5/23, a juvenile became combative and grabbed an aluminum stick-type object. When staff tried to take the object from juvenile, he stated she struck him in the head with it. Staff advised he took possession of the object after the altercation and did not have any injuries.

Staff advised officers that a juvenile pushed a door in his face while being escorted through the facility. In an effort to stop the door from hitting his face, he used his hand to stop the door and his hand was injured. Juvenile was transported by MEMS to acute care for her behavior issues.

Licensing visited the facility to conduct a buildings and grounds walkthrough and to review video of the early morning hours of 8/20/23 – 8/21/23. No deficiencies cited.

Officers responded to the facility for a batter report. Staff 1 reported that on 7/7/23, Juvenile 1 struck him and pulled his hair repeatedly, requiring physical therapy for his neck.
Staff 1 reported that on 8/15/23, Juvenile 2 attacked him with a chair and kicked him on the “privates” after he corrected juvenile for exposing his genitals and making sexual comments/gestures towards staff.
Juvenile 1 attacked Staff 1 again on 8/15/23 by stabbing him on the arm with a pen. Staff 2 stated she attempted to take the pen away from the juvenile, who then struck her in the face and then bit her leg.
Staff 1 advised that there have been several other incidents between 7/7/23 and 8/15/23 and felt it was needed to file a police report for staff safety. Officers advised Staff 1 and Staff 2 on how to obtain a warrant.

A monitor visit was conducted 8/16/23 and video was reviewed of random areas of the facility to check ratios.

The juvenile victim’s grandmother called to report that she was at Children’s Hospital for an injury sustained in a fight at the facility. The victim stated that they suspect threw a chair at her and it hit her nose.

A resident wrapped a pair of pants around her neck, stating “I want to kill myself.” Staff intervened and removed the pants. The facility was cited 907.2 and 907.3 due to staff not being present or supervising an escalated resident for 3 minutes.

Officers responded to the facility for a runaway report. Upon arrival, officer made contact with a staff member who advised that a juvenile ran away from the facility.

A staff member grabbed a resident from the back, pushing him into a bookshelf and making him hit his head while trying to separate the resident from his peers during a physical altercation. The staff member was terminated. The facility was cited 905.4.d, 905.4.g, 109.1.g, and 907.2 as a result of this incident.

A resident grabbed a staff member’s keys, ran through the unit and eloped from the facility. Another resident ran as well but returned with the keys a moment later.

Officers responded to a runaway juvenile report at the facility. Officers located the resident days later and transported the resident back to the facility.

Licensing visited the facility to conduct a buildings and grounds walkthrough. The facility was cited 907.3 for supervision issues observed during the review.

A resident eloped from the facility by grabbing a staff member’s keys and scanning herself off the unit. A staff member followed the resident and stayed with her until police arrived to bring her back to the facility.

Officers were enroute to this call when they observed a female matching the description given. Officers transported the female and an employee of the facility to the facility.

Officers responded to the facility and made contact with a staff member who advised that a juvenile ran away from the facility. The staff member provided a photo of the resident to officers.

A resident grabbed a bottle of cleaning chemicals off a cart and drank some of the liquid. The resident was transported to Arkansas Children’s Hospital for evaluation.

Officers responded to the facility for a missing person report. Officers circulated the area and observed the juvenile at a nearby store. The juvenile refused to go into police custody and advised he would return home at 2300 hours.

A resident reported that a staff member threatened him and shoved him hard. Licensing provided the facility technical assistance for 905.4.g as a result of this incident.

Upon officers’ arrival, staff advised juvenile arrived at facility around 1630 hours and was immediately aggressive and hostile with staff. Juvenile began tearing up property in the lobby during medical assessment and slapped a staff member after she informed another staff to call the police. Juvenile was taken to hospital via MEMS for assessment and replacement.

Officers were advised that 4 van windows were broken out by one of the residents.

Officers made contact with the caller who advised the juvenile had returned prior to the officers’ arrival.

Officers secured the scene for MEMS.

A staff member hit a resident in the face with a clipboard after he “plucked her in the head”. The facility was cited 109.1.g for unprofessional conduct as a result of this incident.

A resident placed a shirt around his neck, pulling it tightly and continued to choke himself until staff members removed the shirt from his neck. The facility was cited 907.2 and 907.3 for issues identified during a video review of the incident.

Licensing visited the facility to review personnel records. The facility was cited 105.15.f, 105.15.g, 105.15.0h, and 107.4 for issues identified during the review.

Licensing provided technical assistance for 102.3.h, because it was determined that the facility’s current policy for “Reporting Instances of Abuse or Neglect” did not comply with Arkansas state law requirements on mandated reporting.

A resident was transported for acute care after tying socks together and wrapping them around her neck. During video review of the incident, a staff member is seen pulling the resident on the floor by her shirt, dragging her down the hallway, and placing her against the wall. The facility was cited 907.2, 907.3, and 109.1.g as a result of this incident.

Officers responded to the facility for a missing person call. While officers were asking questions for the report, the missing resident returned to the facility.
