Licensing Specialist completed buildings and grounds and observed the following areas: comfort room, laundry room, butterfly room, day area, bedrooms, bathrooms, and nurse’S station. The grounds were clean with no debris observed as Licensing Specialist walked to the building. The grounds leading to the gym were well kept. Licensing Specialist observed a picnic table and two (2) benches. Licensing Specialist was escorted by Summer Berryhill, Director of Quality and Risk Management. Residents were not present during the walkthrough. Ms. Berryhill informed Licensing Specialist that the residents were in the educational building. However, the butterfly room was occupied, ratio 1:1. Licensing Specialist observed the following bedrooms and bathrooms: b131, b132, and b133. All bedrooms were organized, and the bathrooms were recently cleaned. Licensing Specialist smelled the aroma of cleaning products. No plastic bags were observed on the unit. B131 had three (3) residents, b132 had two (2) residents, and b133 had three (3) residents. Licensing Specialist observed the following documentation on the walls: evacuation plan, gym time schedule, educational calendar, clients’ rights, grievance, and stress/Fear response system poster. This documentation was located by the milieu and day area. Licensing Specialist reviewed the mar’S on the residents listed below: all initialed and up to date.
