Officer responded to facility in reference to an attempted suicide. Upon arrival, officer observed resident laying on the floor with several nurses attending to resident. Resident appeared to be unconscious but breathing. MEMS transported resident to hospital.
Staff member reported he was making his 15-minute rounds at 2am and resident appeared to be okay, but when he made his rounds again at 2:15am he found the resident on the floor, unconscious, with a pair of yoga pants double knotted around their neck so tight they had to be cut off.
