Resident 1 was in the cafeteria when she became irate over another resident accusing her of inserting herself in their conversation. Resident 1 threw a chair and almost hit Resident 2, who picked up the chair and threw it back at Resident 1. Resident 2 then laid down and kicked Resident 1 in the stomach then in the face. Staff separated the residents.

Resident 2 did not sustain any injuries. Resident 1 was assessed and was bleeding from her nose with a deformity of her nose noted. Resident 1 was sent to the ER for evaluation and was diagnosed with a nasal bone fracture. Resident returned to the facility with orders for neurological checks every 48 hours, no physical activities, bed rest, ice packs, and a referral to an ENT specialist. Residents were separated, placed on assault precautions and unit restriction.

Facility informed Licensing on 1/23/24 that Resident 1 will have to have surgery.
