An officer was dispatched to the facility in response to an assault report. A staff member reported a resident struck him in the face when he was redirected for disrupting class.

Bono Police Department was called to the facility after an altercation between two residents.

After an altercation with a peer, a resident hit a wall unitl his hand started to hurt and swell. An x-ray was ordered.

A resident reported that a peer touched him inappropriately.

A juvenile resident reported pain after an altercation with four of his peers. The resident was evaluated, and an x-ray of his right hand was ordered.

Three juvenile residents were involved in a physical altercation. One resident received an injury to his thumb.

A resident injured his ankle while playing basketball in the gym. The resident was assessed, and an x-ray was ordered.

A resident was transported to the Juvenile Detention Center (JDC) after striking a staff member in the face multiple times.

Officers were dispatched to the facility in response to an assault and battery. One resident was arrested and transported to Juvenile Detention.

A resident approached a staff member and made racial comments. The resident began to damage property and tried to break into a phone box. Bono Police Department was called and arrested the resident. He was transported to Juvenile Detention and discharged from the facility.

Officers were dispatched to the facility in response to a disturbance. A resident was threatening other residents and staff. The resident was transported to Craighead County Juvenile Detention Center.

Officers were dispatched to the facility in response to a resident that was out of control and threatening staff. The staff member in charge at the facility stated they wanted the resident to be arrested for the incident. The resident was arrested and transported to Craighead County Detention Center for processing.

A resident exhibited aggressive behavior, then got into an altercation with 3 peers and threatened facility staff. Staff called Bono Police Department, who arrested the resident and transported him to juvenile detention.

Licensing visited the facility to conduct a Buildings and Grounds review. The following areas were reviewed: Boys Hall, Little Hall, Cafeteria, Indoor/Outdoor Gym, Classrooms. The grounds looked well maintained and free of debris or trash.

Licensing visited the facility to review children’s records. All records reviewed were organized and well maintained.

A resident complained of pain to the right ankle. The resident stated that he rolled his ankle while on pass at the trampoline park. An x-ray was ordered to rule out a fracture.

A resident was struck in the head and face multiple times by two peers. The resident was treated for a laceration under the right eye.

Licensing visited the facility to perform a Buildings and Grounds walkthrough. No deficiencies cited.

A resident eloped from the facility by pushing past a nurse and out the exit door of the kitchen. The resident was located by officers and returned to the facility. After the officers left the facility, the resident began vandalizing property. The officers returned and arrested the resident for criminal mischief.

Officers were dispatched to the facility in response to a missing resident. Officers located the resident and returned him to the facility. After leaving the facility, officers were called back due to the resident vandalizing property. Officers placed the resident under arrest and transported him to Craighead County Detention Center.

An officer was dispatched to facility in response to a juvenile overdose from taking pills.

A resident was transported to St. Bernard’s Hospital by EMS after swallowing an unknown number of pills.

A resident was transported to Northeast Arkansas Baptist Hospital after injuring his ankle while playing basketball.

Officers were dispatched to the facility in response to an assault on a staff member. The resident who assaulted the staff member was arrested and transported to Craighead County Juvenile Detention Center.

A resident committed an unprovoked assault on a staff member by ramming his elbow into staff and slamming him into the wall. EMS was called and the staff member was transported to Northeast Arkansas Baptist Hospital. Bono Police Department was called. The resident was arrested and taken to Craighead County Detention Center.

An x-ray was ordered after a resident reported pain in his hand from hitting a wall.

A resident eloped out of the front office door by pushing past staff when they opened the door leading to the front office. The resident was apprehended by the Bono Police Department and taken to Craighead County Juvenile Detention Center.

A resident reported to his therapist that a staff member made a threatening statement toward him. The staff member was suspended pending the outcome of the internal investigation.

Licensing visited the facility for an unannounced buildings and grounds walkthrough. The facility was cited 911.15.f and 908.9.d for issues identified during the walkthrough.

An x-ray was ordered after a resident injured his left hand by punching walls, windows, and a peer.

Licensing conducted an annual review of policies for the facility. All policies complied with minimum licensing standards.

A resident was injured when a peer jumped up and kneed him in the back, causing the resident to hit his head on the wall. The resident was taken to the emergency room for treatment.

Officer Morris was dispatched to Methodist Family Health in reference to an 11-year-old male client creating a disturbance.

A resident was transported to St. Bernard’s Medical Center after injuring his right hand by striking a wall with his fist.

An officer was dispatched to the facility in response to a report a juvenile was attacking facility staff. The staff member advised that he had been punched in the face by the juvenile multiple times.

Licensing visited the facility to complete a buildings and grounds walkthrough. No deficiencies cited.

Officer Morgan returned a call. The caller advised her grandson had been the victim of sexual misconduct while in the facility.

A guardian of a resident failed to return him to the facility, and the resident was discharged against medical advice. The explanation for the AMA discharge was due to an alleged incident between the resident and a peer at the facility. The complaint was UNFOUNDED by Licensing.

During interviews regarding a maltreatment investigation, resident’s guardian reported to the investigator that approximately 3 weeks ago, she took resident to the dentist. He was bragging to his brothers that he has a 35-year-old girlfriend at the facility (Dacus Psychiatric Residential). Resident stated that they were having sex in the bathrooms because there were no cameras there. Resident reported it to the facility staff, who failed to report it to the director. Staff was terminated from the facility on 12/18/23. CACD investigation found TRUE.

During routine camera view on 12/18/2023 by the nurse, staff was seen pushing and kneeing client in the groin. Client was examined by the nurse. Bilateral testicles slightly red. A call to the child abuse hotline was completed and accepted. Staff was immediately terminated upon view of camera footage. Facility was cited 109.1.g for the unprofessional conduct of staff. The complaint has been FOUNDED by Licensing. CACD investigation found TRUE.

Officers responded to the facility for an aggressive juvenile. A staff member advised that the juvenile punched him in the face and stomach.

Officers were dispatched to the facility in response to a juvenile making threats of killing staff members. Staff advised that the juvenile placed a peer in a headlock and wouldn’t let go. The juvenile advised he and his peer were playing around. The threats toward staff were un-corroborated.

A resident was transported to St. Bernard’s Emergency Department for treatment of a lacerated finger and a dislocated shoulder after he closed his finger in a bathroom door.

A resident was taken to NEA Baptist ER after dislocating his shoulder while playing basketball.

An x-ray was ordered after a resident landed awkwardly on his ankle while playing basketball.

An officer was dispatched to the facility for an out-of-control child. Upon arrival, a staff member advised that the child hit one of the staff members and broke a toilet. The officer spoke to the child who agreed to stay calm and let staff know when he is struggling with a problem.

A resident was sent to the ER after injuring his arm by falling while trying to climb on a basketball hoop.

A resident reported that a staff member brought a THC and nicotine vape into the facility for the resident to use. The resident did test positive for THC. The complaint was UNFOUNDED by Licensing.

A resident disclosed that a staff member brought in a nicotine vape and a THC vape for him to use. The resident tested positive for THC on 10/16/23. The complaint was UNFOUNDED by Licensing.

A resident reported that a staff member brought a phone, and a THC and nicotine vape into the facility and gave them to another resident to use. The resident also reported the staff member instructed her to lie about other staff instead of telling on her. The complaint was UNFOUNDED by Licensing.

Officers were dispatched to the facility in response to an uncontrollable juvenile. A staff member reported that the juvenile punched and bit him.

No deficiencies were noted during the Inpatient Psychiatric Inspection of Care (IOC) conducted on 09/19/23.

A resident was transported for acute care after damaging property and self-harming.

After a restraint, a staff member pushed a resident back, causing the resident to fall to the floor. The facility was cited 109.1.g and 905.4.g as a result of this incident.

A resident was transported to JDC after punching a staff member, threatening to stab someone, hitting a peer in the head with a notebook, and stepping on a peer’s foot.

Officers were dispatched to the facility in response to a juvenile out of control. Staff advised that the resident punched a staff member. The resident was arrested and transported to JDC.

Officers were dispatched to the facility in response to a disruptive juvenile. Officers spoke with the juvenile and calmed him down.

Officers were dispatched to the facility in response to an out-of-control juvenile. The officer spoke to the juvenile who advised the behavior wouldn’t happen again.

After a physical altercation with a peer, a resident wrapped clothing around his neck. Staff removed the clothing.

A resident stated that he wanted to die then took a piece of blue jean material and wrapped it around his neck. Staff removed the fabric.

Licensing conducted an annual review of policies and procedures at the facility.

A resident wrapped an HDMI cord around his neck. Staff removed the cord. Then, the resident attempted to use the metal part from staff’s clipboard to cut his arm. The wound was cleaned and bandaged, and the resident was placed on precautions.

An officer received a call in reference to the caller’s grandson. The caller advised the officer that her grandson was being held there, and she was unable to get a hold of anyone at the facility. The officer relayed the message to a staff member at the facility.

A resident lost a tooth after the resident was punched in the face several times by a peer.

Officers were dispatched to the facility in response to a juvenile hitting a staff member in the face with a closed fist. The officer was informed that the juvenile did the same thing the day before to another staff member.

A resident attacked staff, wrapped a hoodie around his neck, then scratched himself on the arm with a piece of broken plastic.

A resident was placed on suicide precautions after trying to choke himself.

A resident was transported to Northeast Arkansas Baptist ER after falling down the stairs and landing on his face.

A resident wrapped his shirt around his neck in an attempt to choke himself after an altercation with a peer.

An officer interviewed a staff member in response to an incident at the facility. The staff member stated that he was only trying to gain control of the child and take away items that could be used to harm himself or others.

Caller reported she had picked up her grandson today, and when she pulled into the parking lot, the juvenile took off walking, and she lost sight of him. The juvenile was located by Jonesboro PD the same day and transported back to the facility.

Officers made contact with the complainant who reported that a juvenile client punched her in the eye the day prior.

A staff member was observed performing an improper restraint on a resident. The facility was cited 109.1.g and 905.4.g as a result of this incident.

Two residents eloped from the facility by breaking through a gym door and climbing a fence. The residents were returned to the facility by Bono police.

Officers were dispatched in response to two residents who ran away from the facility.

Staff advised officer that a juvenile got upset when he was not allowed to make a phone call and became aggressive with staff. Staff stated he attempted to approach the juvenile to cool the situation down, and the juvenile began swinging at him. Staff stated he grabbed the juvenile and took him to the floor, and the juvenile then bit him on the arm. The juvenile stated he bit staff because of the way staff grabbed him and pulled him to the floor.

Staff advised officer that a juvenile punched him several times in the head as he attempted to keep the juvenile from attacking a peer. The facility was advised to contact the juvenile’s caseworker, as the jail was currently full.
