On 01/04/24 at 12:34PM, resident eloped from facility during fire alarm inspection (strobes and horns). Staff was unaware magnetic locks were also being tested during this time, as they normally would position themselves in front of exit doors during fire alarm drills. Resident eloped out of an unguarded exit door around the side of the building. Line of sight of resident was maintained. Resident returned herself willingly with staff at 12:40pm.

Actions Taken: Resident was medically assessed by Nurse Practitioner Roslyn Perry, APRN on 01/05/24 and reports she sustained no injuries and denies any pain or discomfort. Resident was placed on elopement precautions. West Memphis Police Department was called but was not needed as Resident returned prior to their arrival.
