Resident reported to the Nurse that her throat was burning and confessed to ingesting body wash (liquid soap). The Nurse contacted poison control, MD, and Guardian. Abdominal X-ray was ordered and done in house which revealed no abnormalities or foreign objects. Poison Control informed nurse that the resident may experience some nausea or vomiting. Patient’s vital signs within normal range, no nausea or vomiting. MD ordered resident to be placed on suicide precautions. “She was ordered to wear paper scrubs and on a finger foods only diet. We are currently trying to get her stepped up to a higher level of care on our Acute In patient program.”
On 4/1/24, Licensing Specialist requested safety plan and update on youth’s status. Agency responded: “Her therapist states that Resident denied that it was a suicide attempt and just wanted her Aunt to come get her. Resident’s guardian refused to sign consent for her to be stepped up for acute inpatient and discharged her against medical advice Friday (3/29/24), stating that she wanted her to receive outpatient treatment.”
