On 11/26/23 Staff1 was approaching Resident1 in a threatening manner when another staff member intercepted. Simultaneously another resident (Resident2) ran and began hitting Staff1. A third staff member attempts to separate Resident2 from Staff1. Staff1 is seen reaching over the staff members attempting to strike Resident2′ s head and appears to have pulled her hair. Unable to determine if the strikes made contact. Resident did not sustain any physical injuries. Staff1 was removed from the facility immediately following this incident and termed the following day on 11/27/23. *This incident was reviewed in greater detail via camera footage 12/7/23. Prior to this date it was not discovered that Staff1 was attempting to strike Resident2. Report was made and this incident is currently under investigation.
Licensing Specialist: Kendra Rice
Licensing spoke with Ms. Berryhill, Director of Quality and Risk Mgmt. Berryhill reported that camera footage was observed on 11/27/23 and staff member involved was terminated due to behavior. Keys were taken and she was removed from the facility. On 12/7/23, the Youth Coordinator reported that the staff member involved in this complaint might have hit the resident. Berryhill reviewed the camera footage again from different angles.
Facility will be cited for 109.1g. Staff member was observed charging at a resident in an aggressive manner. 905.4g, Staff member was observed trying to reach over other staff to grab resident.
CHRIS indicated case was found True. Complaint has been Founded by Licensing. Berryhill informed Licensing that staff has been retrained on mandatory reporting.
