- Client reported that a staff member grabbed his arm and pinned him against the wall. Client was unable to provide a date/time when the incident occurred.
- Child Abuse Hotline was called and accepted. Staff was placed on leave pending the outcome of investigation. Specialist was at facility for buildings and grounds visit on 11/30/21; Staff stated they reviewed video of the past couple weeks and did not see any video where client and staff were engaged in any activity as mentioned. Specialist visited facility on 12/6/21 and reviewed camera footage. Facility has since terminated staff mentioned in the incident.

- Licensing Specialist received notice from administrative staff that 2 calls were made to the hotline by Center’s staff. Neither call involve staff members. The first call was accepted by the hotline: Client told staff that when she was in Ft. Smith, AR, (sometime in Sept. 2021) an unknown male (35 y/o) took her to a park and forced her to perform oral sex on him. The second call was accepted by the hotline: Client told staff that 2 boys touched her vagina and breasts and made her touch their private parts during her time at 2 other facilities (Youth Home and Methodist Behavioral Health). Client also told staff that her mother made her “strip” and touched her (client) between her legs. Client also told staff that sometime in 2021, she exchanged nude photos with an unknown man in Canada over the internet; This information was reported to the Internet Crimes Against Children Hotline on 11/17/21. All reported incidents occurred prior to client being admitted to Centers facility (Intake Date: 10/14/21).

- N 188 – Facility failed to ensure all staff involved in an ESI participated in a debriefing for 1 client who required the use of a personal restraint.

- Plan of Correction –
- Retraining on ESI documentation and 12th Street Crisis Management Protocol; ESI forms will be reviewed daily.
- During the Revisit Survey conducted on 12/15/21, the facility was found to be in compliance with program requirements.

Staff stated Juvenile got upset when staff pointed out the misuse of phone privileges and yelled and pushed staff. When staff tried to restrain Juvenile, he punched staff in the face. Staff advised the cameras were not working, and the facility has started in investigation. Staff wanted a report made.

- During call time for client, time was up for his call and client became escalated and took off his coat and shirt. Staff member grabbed client, leaving red marks and some bruising on client’s shoulder and lower parts of his neck.
- Child Abuse Hotline was called and accepted. Staff member was terminated, and other staff viewed on the video have been retrained. Specialist completed a visit on 11/16/21 to provide a 521 with citations for regulations 907.2 and 905.4g.

- Client was transported to ER after stating she took 10 pills that she had regurgitated and put in a book. ER staff stated there were no signs of client taking extra medications due to tests/vital signs. Staff at facility stated client has been on med watch and she has been sitting for 30 minutes after taking medication and mouth is checked before she is able to return to activities.
- Client was sent to acute; Client has been on and will stay on med watch and tongue dispenser is inserted in mouth when medication is taken.

- R911.15.d – Shorts with string that could be taken out and used for self-harm was removed during walkthrough. Client was new to the program and Director stated they were just missed.
- Checked MARs for 3 residents with no concerns noted. EMCC was not visited due to outbreak of lice. EMAC was not visited due to incident.

Staff stated J1, J2, and J3 were in a verbal altercation and were rough with staff when they began to intervene. The juveniles made statements of self-harm; MEMS were advised to respond. Decision was made to keep juveniles on the property.

- Client stated to staff that he had sex with another client while the 2 were at another facility together. This was reported to Child Abuse Hotline and accepted (referral # unknown).
- Clients are being assigned to separate dorms, classrooms, and group sessions. Staff have been informed to keep clients separated during outside time, meal services, and other activities.

Staff advised Juvenile 1 attacked Juvenile 2 in class. Juvenile 2 stated he was talking to himself after Juvenile 1 told him to stop talking to him, and Juvenile 1 got up and put his hands on him. Officers observed small knot on Juvenile 2s forehead.
