  • 103 – no out-of-state check for one staff
  • TA – ensure bedding is changed in one bed in east dorm, removed during visit. Viewed camera footage from 3/8/20 incident – no MLS violations, discussed regular residential and psychiatric clients sharing bedrooms (no reg but discouraged).

Staff stated Juvenile 1 was verbally loud and started punching windows and a thermostat. Because of this, Juvenile 2 got agitated and upset with Juvenile 1 and started screaming and yelling. Juvenile 1 stated he held Juvenile 2 back from being further violent with Staff. Staff stated both Juveniles separated on their own. MEMS responded but were not needed, Medical Staff stated they will deal with both Juveniles later. No injuries reported.

Staff reported that resident stated she had hoarded her medication for four days and took it all at once; unknown if resident actually took medication; resident in route to ER, staff following MEMS.

Staff reported that resident damaged water sprinkler system in attempt to “free some of the other residents”; staff stated no charges will be filed due to residents mental capacity.

Officers made contact with staff at facility; After checking with everyone, staff advised that everything was fine and police were not needed.

Officers responded to an overdose, unknown on how resident obtained medication taken; resident transported to ER.

Staff requested assistance in securing aggressive resident threatening to self harm; officers made contact and noted resident did not show any signs of aggression; MEMS transported resident to hospital without incident.

Staff was picking resident up after an altercation with her teacher; resident refused to get in vehicle and walked away; officers searched area, resident not located; resident returned to facility approx. 2 hours 14 minutes after reported missing; released to DHS custody.

Staff stepped between two residents that were fighting and sustained a few minor scratches to face; officers advised staff to seek warrant for battery.

Staff reported that a resident stole a phone and called her “pimp”, resident stated that he is a blood gang member and will come “shoot up” the facility; officer advised staff to lock all doors and to call back if anything suspicious happens.

Resident throwing chair and books; teacher tried to assist staff in restraining resident when resident slapped and punched teacher in face; no marks noted on teacher; no charges filed due to residents age.

Officers secured the scene on a suicidal Juvenile. Juvenile was not combative and agreed to being transported by MEMS; MEMS transported Juvenile to Pinnacle Pointe Hospital without incident.

Officers responded to assist MEMS with a transport; Juvenile refusing to go to Pinnacle Point. After Juvenile dropped to ground and laid there, Officers picked up Juvenile and placed him on gurney. Officers followed MEMS to Pinnacle Point where Juvenile was dropped off.

Staff stated she was picking up Juvenile from a doctors appointment; When Juvenile came out she was upset and walked away from Staff. Officers located Juvenile and brought her back to staff; Staff stated Juvenile needed to be taken to Pinnacle Point. Juvenile was taken to Pinnacle Point without incident.
