QRTP – Staff stated J1 hit J2 with belt. J2 stated J1 started hitting him with a belt, and when he tried to get the belt away, J1 began to hit him in his ribs. Staff stated they have had issues with J1 lately.

- Suicidal/Disturbance
- Client attempted to scratch arm with container lid, and then started picking at sores. Client then started banging head on door and tried to take strings from her mask and tie them together. Client became physically aggressive when staff intervened and scratched/hit staff; Client punched doors/walls. Client knocked a screw out of the door and placed it in her mouth; Staff was able to get screw from her mouth. Client then placed a bag over her head, swung at staff, and was then placed in a physical hold and received a chemical restraint.
- Video was watched and everything was done correctly except staff was heard on video stating that she would take away food as a punishment. Agency was cited and staff will be retrained.

QRTP – Staff stated they heard a loud commotion at end of hall and located J1 in J2 and J3s room. J1s nose was bleeding and had been fighting J2 who accused him of stealing money. J1 also stated that morning, J2 and J3 were fighting. J2 stated he did not steal J1s wallet, but later returned it to officer to give to J1. J3 stated after J1 was calling him a pedophile and hitting him, he picked up J1 and slammed him on the ground. J3 had a busted lip.

Staff advised Juvenile ran away from facility after attacking staff and stealing keys. Staff stated Juvenile became upset when he asked her to sit down, threw her shoes at him, stole his keys, and used them to leave the building. Officers circulated the area with negative results. On 11/17/21, Conway PD advised they have the Juvenile and are contacting DHS to pick up Juvenile.
