Methodist Children’s Home operates multiple programs on a single campus. Police responses to this campus often do not identify the program they are responding to. All reports plausibly related to the PRTF or Qualified Residential Treatment Program/Group Home (QRTP) have been included here. Where we can be certain the response was to the QRTP it is noted.

Contact with family (Calls and visit schedule): Clients have regularly scheduled phone call opportunities. The number of actual calls depends on the clients wish to utilize these opportunities. In person visitation (assuming no quarantine precautions) is available every weekend and can be scheduled at other times as needed by the family or guardian.

Police Report

Disturbance

QRTP – Staff advised a juvenile was picking up rocks and metal items and throwing them. Upon arrival, officer placed juvenile in patrol vehicle while staff determined new placement for juvenile. Juvenile was transported to acute hospital.

  • Maltreatment
    • Client was provoking a female staff, walking into her personal space many times. The staff member pushed client back and then left the floor. The Child Abuse Hotline was called, unknown if accepted. Corrective Action: The staff member will be re-trained on CPI and will be moved to the girl’s unit.
    • Licensing follow-up: Licensing inquired when staff will begin working on unit and the date of the next in-service (for CPI). Licensing visited the facility on 12/27/22 and viewed video footage. Licensing observed the staff member and client arguing back and forth as staff was trying to get client to return to his room until it was his turn to complete hygiene. The client approached staff, and staff could be heard informing client that he was in her personal space. After moving away from client several times, staff pushed the client before being separated by other staff.
    • Facility was cited for 109.1.g (staff engaged in unprofessional conduct when she pushed the resident).

  • Licensing Specialist: Clayton DeBoer.
  • Buildings & Grounds conducted from 10am – 11am. Census: 27. Hall B observed. MARs checked for 5 residents.

Terroristic Threatening First Degree, Terroristic Threatening Second Degree

QRTP – Staff stated J1 began causing a disturbance after getting suspended from school. Staff observed J1 threaten J2 & J3, stating he was going to beat their ass and also threatened to kill J2. J1 told officer he was just playing and did not intend to harm peers.

Staff member stated while at work, juvenile became upset and began throwing hygiene items at him for no reason. After juvenile began to throw chairs at him, he restrained juvenile per facility policy. The Child Abuse Hotline was contacted.

  • Maltreatment
    • Staff and residents entered the cafeteria for breakfast, and staff member threw his clipboard and drink on a table. After another staff spoke to clients regarding rules, the staff member stated to everyone that if someone else throws anything else his way, he will be throwing it back at them. Client tried to express that she wasn’t throwing anything at him, but staff proceeded to slam his hand on the table and repeated his statement several times. Another staff called a five second rule to remove all other clients, while the staff member and client are still going back and forth. Staff member continued to antagonize client and provoke her to throw something at him as another staff entered the cafeteria, and the client escalated and threw a chair towards the staff member. The staff member then picked up a chair and threw it towards client and the other staff. Client threw another chair at the staff member as he was walking towards her, and they began tussling. Staff member is seen grabbing client by the hair and dragging her on the floor. The Child Abuse Hotline was called and accepted. The staff member was terminated.
    • Licensing follow-up: Facility visited on 11/23/22 and Licensing spoke with staff. Video was not available due to internet being down. Licensing visited the facility on 11/28/22 and viewed video footage. Licensing viewed the staff member throwing a chair at the client, then grabbing her hair and throwing her to the ground before being separated by other staff. Staff is also heard shouting profanity during incident.
    • Facility was cited for 109.1g, 905.4c, 905.4g, and 907.2.

QRTP – Staff informed officer that juvenile broke the back window of a work van. Juvenile told officer he broke the window because he was tired of being hit by other kids at facility.

    • Client received an x-ray of her right hand due to pain after she punched a wall.
    • Licensing follow-up: Client’s hand x-rayed due to pain.

    • Client became upset due to an altercation between clients and punched the unit door. Nurse noted swelling to client’s hand, and an x-ray was ordered.
    • Licensing follow-up: An x-ray of client’s hand revealed no fracture.

QRTP – Staff informed officer that juvenile had been disruptive in the transport van. While at school, juvenile had broken an ink pen and pulled the safety pin out of a fire extinguisher (it was taken away before he could activate it). A facility in Maumelle had a bed ready, and MEMS arrived and transported juvenile to ACH first for evaluation.

Licensing Follow Up, Maltreatment, Notice of Incident

  • Licensing (Maltreatment)
    • A verbal altercation began between a client and staff member. Client then walked into staff’s personal space, and a push was initiated by staff. Client attempted to attack and hit staff, and staff attempted to grab client’s arm from striking her. Staff initiated a restraint but was unsuccessful. Both staff member and client fell into the couch area, and other staff came to assist and the involved staff member tapped out. CAP: An in-service on the correct CPI holds, personal space between staff and clients, a write-up about voice tone/language towards clients, and the staff member will be moved from the girl’s floor to the boy’s floor. Child Abuse Hotline was called, not accepted.
    • Licensing follow-up: Facility visited on 10/28/22 and camera footage reviewed. Client is not viewed or heard being a threat to herself or anyone else. Staff member walks towards client and puts her chest directly against client, then shoves client to the ground. Staff attempted to grab client, and a short altercation followed before staff exited the area. Facility cited 109.1g, 905.4g, and 907.2. Licensing made another report to the Child Abuse Hotline and it was not accepted. The staff member involved will be retrained and will receive a written warning and a Performance Improvement Plan.

    • Client received an x-ray of his left foot due to pain from doing karate moves and jumping from furniture to furniture.
    • Licensing follow-up: No licensing concerns noted.

QRTP – Staff advised officers that juvenile was throwing a temper tantrum. Staff stated juvenile did not throw any bricks, but was walking outside and not obeying her. Staff advised this happens multiple times throughout the week and no facility will accept him. After speaking with officers, juvenile calmed down and went back inside.

Staff advised officers that on 10/3/22, suspect arrived on the property after staff called him to pick up his child due to medical reasons. Staff advised suspect became upset and charged at her after arriving. Suspect was stopped by another subject at facility, and did not touch staff.

QRTP – Upon officers arrival, staff stated juvenile locked himself in the main office after refusing to go to sleep. After staff got the door open, juvenile started swinging around a bobby pin, then walked out of the building. Officers made contact with juvenile and retrieved the bobby pin, then transported him back to the facility. His caseworker advised staff to call hospital for acute placement; Staff advised officers that juvenile had just returned from the hospital 2 hrs ago, and they would not have a bed due to juveniles past violent history with them. Juvenile went to his bedroom without incident.

QRTP – Upon officers arrival, staff stated juvenile was threatening the residents and staff, stating he was going to kill them and himself. Juvenile also went outside and put his fist through the window pane and wrote the word “bitch” on the front door in his own blood. Juvenile was transported to hospital via MEMS.
