Capacity: 162 (111 Arkansas Medicaid PRTF beds, 51 Residential beds used for out-of-state resident PRTF placement or AR contract beds).

  • ER Visit (Accidental Injury)
    • Client was playing recreational game when he fell and hurt his arm. Client was taken to Dallas Co. ER, then to ACH for treatment. AR Children’s Hospital: Closed fracture of distal end of left radius, unspecified fracture morphology. Closed reduction performed under deep sedation. Agency will continue to monitor client.

Staff requested officer to Millcreek (Magnolia Hall) due to juveniles hitting staff and 3 juveniles missing from facility. Upon officers arrival, there was no disturbance in halls and they found the missing boys.

  • Maltreatment
    • Alleged staff member aggressively yelled at client and intentionally hit him on his head 4 times (unknown what staff used to hit client). No injuries reported. There is a voice recording of the staff member yelling at client.
    • Child Abuse Hotline was called and accepted. Staff member was placed on administrative leave pending investigation. Facility visited on 3/8/22 and video reviewed. Video did not show staff member hitting any children but did show staff member point her finger and appear to yell (no audio). Client and staff statements were reviewed. Staff denied hitting client but admits she yelled at client to go to bed. Two other clients were interviewed and did not indicate staff had ever hit them. Client involved indicated staff member hits and slaps him in the back of his head and grabs his neck. When asked if staff ever called the client names, client stated “FB, MF, SOB”. Client is believable when asked to describe how staff hits him. Risk Management stated the staff member has been involved in a previous incident and is on leave and to be terminated. Facility cited (905.4g and 907.2); Staff member terminated.

Staff advised officers that some of the residents were trying to fight with the staff (at Magnolia Hall). Staff advised they had the situation contained at this time but if officer could show his presence to keep the residents from acting out again. Officer stayed on scene while they had several residents change rooms.

ICF – Staff reported a staff member was hitting on a juvenile, and that the juvenile had told staff about the incident (occurred on 2/13/22). Staff stated she called the Hotline and they took the report.

Staff reported juvenile was fighting peer and staff stepped in to break up the fight. Staff stated when staff tried breaking up the fight, the juvenile turned around trying to fight her. Staff stated she called the Hotline but they refused the report because it was resident against resident.

On 2/14/22 at approximately 2259 hours, officer was dispatched to facility for escapee. Staff stated juvenile was last seen around 2215 hours. On 2/15/22, staff advised PD that juvenile had returned around 0623 that morning.

ICF – Staff called to report that they had contacted the hotline about an abuse allegation on a Millcreek employee. Allegation accepted by hotline. Staff stated they did not need a report filed.

  • Maltreatment
    • It was reported that alleged staff member was looking for an object (possibly a pencil or pen) and although client was holding out her hands showing staff the object, staff member still put her hands down in client’s bra. It is unknown whether this was done for sexual gratification or not, but staff member has made it known to the kids that she is bisexual or lesbian. Client has behavioral issues and was violently raped when she was younger and due to this incident, has been having flashbacks of when she was raped. About a month ago, 2 female staff members were trying to restrain client; Staff had client’s hands behind her back while the other choked her. It is unknown who those staff members were, but a report was made to HR and the women quit their jobs. Client was told it was her fault that they quit. Client’s time on the phone has been cut down to where she can only talk for 5 minutes with staff there the whole time. Report was made to law enforcement concerning this.
    • Child Abuse Hotline was called and accepted. Facility visited; Reports in client’s file for previous incident (12/19/21 according to file) and incident on 2/6/22 yield no licensing concerns. Client has a history of self-harm behavior, and staff member reported to have removed the object from client’s bra to prevent injury. CACD investigation unsubstantiated.

Staff advised officer at approximately 2300 that juvenile ran away from Oak Creek on Cadiz St. Staff called back at approximately 2341 and requested to know when the juvenile was found.

At approximately 1738, staff requested officer to facility for 3 runaway juveniles. At approximately 1932 hours, officer was advised by dispatch that all juveniles had been apprehended by facility staff.

  • Maltreatment
    • Client was oppositional and disruptive most of the day; After entering her room, she was physically assaulted by a peer, resulting in a bloody nose and 2 chipped teeth. Client alleged that staff member and supervisor encouraged the peer to assault her and did not prevent peer from entering her room. Nurse evaluation: Pain reported to nose and mouth area at 7/10; Two chipped teeth with 2-3 loose teeth. Client was transported to ER for assessment and referred to dentist for dental work. No fractures to nose.
    • Child Abuse Hotline was called and accepted. Staff were placed on administrative leave and in-service completed. Each client has been moved to another unit. Facility reported that multiple versions of events had been stated by different staff and clients. CACD investigation substantiated.

Staff called to report a runaway juvenile at 2036, stating he had ran away about 10-15 minutes prior. At 2100, staff advised they had caught the juvenile.

ICF – On 1/11/22, unknown caller from Millcreek reported that someone was stealing money and hung up. On 1/19/22, staff called to report she had an employee that stole money from children at the facility. Staff member stated that LR got approval from clients to use some of their money for Christmas gifts for them. Staff stated when they inspected the clients rooms, they didn’t find any of the items that were supposedly purchased for them. Difference in money spent and returned: Client 1 ($37.19); Client 2 ($329.70); Client 3 ($522.71); Client 4 ($141.97). On 1/20/22, Officer made contact with Program Director, who stated Millcreek reimbursed client’s money on 1/17/22. Program Director stated in Dec 2021, he advised all employees that had taken money out for residents gifts needs to return all unused money so he could close out the books for the year. All staff complied except for LR. Staff stated the only client’s money that was taken and no gifts were purchased nor the money returned was in the amount of $3500.00. Millcreek reimbursed client’s money on 1/17/22. On 1/28/22, Officer had a felony warrant signed for LR for Theft of Property.

  • Maltreatment
    • Client stated to his therapist that peer touches him in his private area and made the comment, “I’m going to molest you.” Peer stated the allegation was fabricated to get him in trouble and gain access to his Nintendo gaming system.
    • Child Abuse Hotline was called, not accepted. Clients have been separated pending internal investigation. No follow-up details.

    • Client reported to staff that staff member picked him up and slammed him on the floor in another client’s bedroom. Nursing evaluation: Nickle size abrasion to left elbow with superficial scab present. Child Abuse Hotline was called and accepted. Staff placed on administrative leave.
    • Licensing follow-up: Facility visited 1/6/22. No camera footage available of incident. Facility cited (110.12: Incident occurred on 12/31/21 and hotline was called and accepted; Licensing was notified on 1/5/22). CACD investigation unsubstantiated. Staff was retrained and brought back, but no longer works at facility for unrelated reasons.

  • Maltreatment
    • Client reported that peer was “feeling her breast and rubbing her elbow and tried to hold her hand” while their group was watching a movie. Client reported during the internal investigation that both girls were fully clothed. The event was alleged to have occurred over a period of 3 minutes. Nurse evaluation: No injury indicated.
    • Child Abuse Hotline was called, not accepted. Specialist followed up on 12/28/21: Staff stated they would research what staff ratio and staff names were during incident. Staff ratio reported to be within licensing standards. No camera footage of area or incident.

  • Maltreatment
    • Client alleged that staff member “bitch slapped” him in the face while intervening in an episode of physical aggression. The primary interview with client showed discrepancies in the alleged events that lead to doubts of the authenticity of the report. A follow-up interview on 12/21/21 matched the original allegation. Interviews conducted with 2 staff and 4 patients revealed that no eyewitnesses corroborate the statements of client being slapped. Nurse evaluation: No redness, swelling, or bruising to the indicated area.
    • Child Abuse Hotline was called and accepted. Staff was placed on leave pending outcome of investigation. Specialist followed up on 12/21/21: Facility’s internal investigation yielded nothing to substantiate report. No camera footage to review for licensing standards.

Staff reported missing juveniles at 1739. Staff could not provide complete descriptions. Upon arrival, staff stated they had both subjects in custody (at approximately 1755).

Staff reported missing juvenile at 1755 and stated they didn’t know what time he left. Juvenile was located by police at 1841 and was picked up by staff.

  • Maltreatment
    • Client stated that staff member instigated a group of 4 fellow patients to fight her. Internal investigation conducted with 3 staff and 5 patients revealed that no witnesses corroborate the statements of client. It was admitted by alleged staff member that on 12/4/21, client got in her face and slightly pushed her, in which staff pushed client’s shoulder and told her to move. Nurse evaluation: Scratch under and over left eye. Scant amount of bleeding. Small knot to left side of forehead with several scratches. Client had hair pulled out on both sides of head with scant bleeding noted.
    • Child Abuse Hotline was called, not accepted. Staff placed on leave. Specialist followed up via phone call on 12/9/21: Staff emailed the hotline call reference number and stated it had not been accepted. Moving forward, staff will be properly trained that physical injury or threat of bodily harm may not be used when interacting with clients. Facility visited on 12/15/21; Facility cited for 905.4g. Facility cited for staff pushing client.

  • Maltreatment
    • Client reported to his therapist that staff member was trying to protect peer during a physical altercation they had and in the process of separating both patients, staff member slapped client. Client stated he was punched in the left cheek multiple times by peer. Nurse evaluation: Slight discolored area to left cheek bone.
    • Child Abuse Hotline called and accepted. Specialist followed up on 11/18/21: Staff stated clients and staff were interviewed and no camera footage was available of room where incident happened (bedroom). Video of outside of bedroom is being held until staff can view. Staff member has been placed on administrative leave pending outcome of investigation.
