Nurse at facility advised officers that Juvenile had ran away from facility and gave a description. Dispatch was advised that a Dallas County Deputy had caught Juvenile and transported them back to facility.

Officer responded to facility for a report of up to 3 runaways. While en route to facility, officer picked up 2 of the 3 residents that had ran away that were walking on the side of the road. When back at the facility, 1 of the residents became belligerent and combative and started threatening officer, stating they would kill officer and fight staff and residents at Millcreek. That resident was transported to the Fordye Police Department (later was transported back to facility same day). On a later call that day, dispatch advised that the 3rd resident had returned to the facility.

Officer responded to West 2nd St. area for a runaway juvenile that was reported. Upon arrival, officers made contact with juvenile who stated they ran away because staff wouldn’t let them plug in their stereo. Officer transported juvenile back to facility.

Director at Facility advised officers he needed to make an abuse report that he had a staff member drag a patient out of a room, down the hall, and into another room by his feet (on 12/31/2019 on 3pm11pm shift). Director stated there was also another staff member present that did not report this; They found out about the incident when the patient requested to see a nurse the next morning. Resident had bruises and scratches that match what happened. Director reported incident to Arkansas Child Abuse Hotline, the staff member was put on immediate Administrative Leave.

Caller requested Officer for a runaway Juvenile; Caller stated 5 Juveniles had ran away and they had found 4 of them but 1 was still missing. By the time Officer was on scene Staff had found last missing Juvenile. Caller requested a welfare check on his daughter at facility because she had been beat up by a couple of peers.

Caller stated a Juvenile had ran away from facility shortly after leaving for school. Juvenile was last seen around 0755 in the morning after taking his medication. Officer located Juvenile and transported him back to facility to Staff.

At approximately 2207 hours Officer located 2 of the 4 Juveniles that had ran away and transported them back to facility to Staff. At approximately 2328 hours, the other 2 Juveniles were found and transported back to facility as well.
