Capacity: 162 (111 Arkansas Medicaid PRTF beds, 51 Residential beds used for out-of-state resident PRTF placement or AR contract beds).

  • Regs out of compliance –
    • W 104 Governing Body –
      • Multiple areas of missing or chipped paint.  
      • Paneling pulled away from wall. Cabinet door missing.
      • Dark, unknown substance on bathroom counter and walls.
      • Gouge in sheetrock by bed in bedroom.
      • Air return vent damaged and bent, heating vent bent down into the duct work. Multiple vents and light fixtures had dirt and dust in them.
      • Exit sign loose with screw exposed, vent cover loose with screw exposed.
      • Multiple broken tiles on counter and floor, some with discoloration.
      • Cooking oil bin outside had buildup of black greasy substance.
      • Edging came off plexiglass window, and tape was used to keep snow out.
      • Edging/trim missing in multiple areas.
      • Towel was placed at bottom of shelf inside refrigerator due to leakage.
      • Bottom of cabinet which contained pots and pans was discolored with a black substance and had a strong, moldy smell.
      • Linoleum coming loose or missing from several areas.
      • Pipe had hole in it with aluminum foil stuffed in the hole.
      • Dishwasher had buildup of unknown brown substance.
      • Chairs/couches had multiple cracks, holes, and tears present.
      • Equipment in backyard area in disrepair with no safety signs.
      • Multiple mattresses had indentions, approximately 9 needs replacing.
    • W 159 QIDP –
      • No recent QIDP observations for several clients.
    • W 341 Nursing Services –
      • Covid 19 screening/temperatures not consistently recorded and checked.
      • Masks not consistently being worn.
    • W 371 Drug Administration –
      • Meds were administered to clients without asking questions or giving clients information related to medication.
    • W 454 Infection Control –
      • Food in freezer/refrigerator unlabeled and undated, bottles with caps missing.

Escape from Youth Service Program, Second Degree Escape

Officer received a call on 5/22/21 at approximately 1524 hours that 2 Juveniles had left the facility. On 5/23/21 at approximately 1815 hours, Officer was advised that the 2 Juveniles had been located by Jefferson County Sheriff’s Office; Jefferson County stated they would hold the Juveniles until they could be transported back to facility. Facility advised someone would be on the way to get both Juveniles.

  • Staff Abuse
    • Resident reported staff pushed her against the wall scratching her arm and attempted to pull a pillowcase out of her hands. The resident then began to punch the staff member and the staff member hit her in the eye as other staff were separating them.

Escape from a Juvenile Detention Facility, Second Degree Escape

Officer was dispatched at approximately 1021 hours in reference to 3 Juveniles that escaped the facility. Upon arrival, Staff stated they had caught 1 of the Juveniles. Officer patrolled the area and located the other 2 Juveniles, transporting them back to facility.

Officer was dispatched at approximately 1922 hours in reference to 2 Juveniles that ran from facility towards nearby woods. Officers were unable to locate. Officer spoke with Staff on 2/27/2021 who stated that both Juveniles were found on 2/25/2021, one at 8:30pm and the other at 11:30pm.

    • During an individual session with his therapist, resident reported staff member slapped him hard in the face twice in his room before breakfast.
    • Staff placed on leave pending investigation.

Officer was dispatched at approximately 1358 hours in reference to a runaway Juvenile. Staff stated Juvenile ran towards the woods and there are other employees out looking for him. Officer was advised at 1450 hours that Staff had the Juvenile back in their custody.

    • 911.6, 911.15.f (Flamingo Hall) – Shower knob broken and loose, hand dryer vent broken with sharp/jagged edges (Compliance date 2/16/21 for both). Room had drawstrings in clothing (Compliance date 2/9/21)
    • 912.6, 911.6 (Tiger Hall) – Soap dispensers empty (Compliance date 2/9/21). Ceiling tile missing in hallway (Compliance date 2/16/21).
    • 911.15.f (Eagle Hall) – Bandana in room 608 and drawstring in room 604 (Compliance date 2/9/21).
    • 911.15.f, 911.6, 908.8 (Kangaroo Hall) – Drawstring in room 407, toilet clogged, Surveillance/breaker box room not locked/secured (Compliance date 2/9/21 for all). Broken handle on hallway door, Nurses were pre-logging medication for their shift (Compliance date 2/16/21 for both).

Incident Report

  • IC stated that a peer told him that he wouldn’t “run 30 seconds” with another patient he would be attacked by others. The patients went into room and proceeded to do the 30 second fight. Staff found out about the situation and notified the nurse and supervisor.
    • Injuries: 1” laceration to left eyebrow, bilateral swelling and bruising to cheeks, abrasion to right cheek. Sent to ER – 3 sutures to left eyebrow.

  • Staff completing 15-minute observation rounds witnessed resident appearing to have seizure in her room. Resident taken to ER.
  • 9/11/20 visit items corrected.

Disorderly Conduct

Upon Officer’s arrival to facility, there were several employees outside talking with another individual who appeared upset. Individual stated he was upset about his money from working at the facility. Officer advised him that he could no longer stay at facility due to them asking him to leave and he is no longer working at the facility. Individual calmed down and Staff advised him of his money that would hit his bank account on the next pay period. Individual apologized and left on his own.

Assist Other Agency, Missing Person

Officer was dispatched at approximately 0800 hours to abandoned house for a missing child called in the previous evening by staff member. Missing since approximately 2230 hours. Officer could not locate. At approximately 1640 hours on 1/10/21 Officer was informed Juvenile had been found and returned back to facility. Juvenile had been sleeping in a vehicle on an employee’s mother’s property.

  • 4 peers attacked another resident after he continued to threaten to try and get a staff member fired. Laceration on lip, sent to DCMC-ER, received 8-10 stitches to bottom lip.
  • Interim POC – IC was moved to a different unit.
  • History: 1 founded maltreatment in last year, 18 founded licensing complaints in last year.

  • IC upset she got put on UR. IC was fighting with Peer. IC started kicking on the door triggering another client. Client went up and hit IC. IC had abrasion to left lower arm area and “bleeding to first finger near hair area but no injuries from altercation.”
  • No interim or final POC or PRLU follow up listed.

Staff stated that they had reports of a gun in the area. The gun was located.

Staff reported 2 Residents ran away from facility. Both Residents were found a little over an hour after reporting.

Staff reported that a Resident might be in the ceiling, and maintenance was currently on the their way to locate individual. Staff reported that Resident was hiding in the ceiling, but they would notify officers if Resident tried to leave facility.
