Capacity: 162 (111 Arkansas Medicaid PRTF beds, 51 Residential beds used for out-of-state resident PRTF placement or AR contract beds).

Contact with family (Calls and visit schedule): Family visits vary on a case-by-case basis. Phone Calls are allowed on 4 days of each week and are limited to 10 minutes unless otherwise specified.

Battery Third Degree

Upon arrival, Juvenile stated that her area had bed bugs and that a staff member pushed her down to the floor causing a bump on her head. Staff stated they were unaware of any incident and that Juvenile has stated several times if she calls the police they would take her to a different place to stay. Officer called the Child Abuse Hotline, it was not accepted.

    • R908.8 – Reviewed med log and noticed clients 8am med for 5/27/21 hadn’t been initialed. Nurse advised she had given it to him but forgot to initial.
    • 911.15.f – Observed shoestrings in 4 different rooms and dayroom, student with strings in hoodie, drawstrings in clothing room 403. Compliance date 5.27.21 for all.
    • 907.2 – Observed client asleep in room and unattended by staff in Kangaroo Hall room 408.
    • 911.6 – Hole in wall outside Nurse’s station, broken bathroom sink, gazebo roof with exposed nails. Compliance date 5.27.21.
    • 911.6 – Trim missing and peeling in rooms 2 & 4, holes in wall in rooms 8 & 9. Painting (agency is working on painting), Hall 2 bathrooms missing shower head. Plate missing in main area. Compliance date 6.27.21. (LS Felicia Moore)

  • Maltreatment – Staff Abuse
    • Client reported that 2 weeks ago, staff choked him in the hall; Client stated this has happened several times. Nurse assessed client: Scratch, scabbed over to area below right eye; faint purple area to back of left arm; pain rated as 0/10. Staff placed on leave pending investigation.
    • Child Abuse Hotline was called and accepted. No footage available since incident occurred in back hallway with no camera. Investigation still pending.

  • Maltreatment – Staff Abuse
    • Client reported staff member slapped her and then directed a peer to fight her. Bruises on arms and face. Staff member placed on leave pending investigation.
    • Child Abuse Hotline was called and accepted. After video footage request, Staff reported there was a power outage during the timeframe of incident, so no camera footage was available. Incident still under investigation.

Staff stated they did not need an officer to come out, she just needed to make a report that they had a fire on 5/15/21 at one of their houses. Staff advised the fire department came and put the fire out.

  • Injury
    • Client tripped over tree root while playing soccer causing him to fall to ground; Complained about his knee and ankle hurting. Client sent to DCMC-ER for further evaluation.
    • Radiology Report: Oblique fracture through the distal fibula at the syndesmosis. Returned with instructions to take Tylenol or Motrin as needed and to use crutches.

Juvenile Protection Officer from Nevada advised that she wanted to have an officer go and talk to Juvenile because she had made some allegations against staff for sexual assault; No other info.

Battery Third Degree

Staff stated that on 5/6/21, a patient reported that a staff member had hit him in the head and held his head against the bed frame afterwards. Staff stated she called the abuse hotline on 5/6/21 and they wouldn’t accept the report. Staff stated she talked with the staff member who was accused, and the staff member stated it didn’t happen but that there was an altercation between him (the patient) and other patients. Staff stated she had put the accused staff member on leave until she could finish her investigation. Staff stated there were no marks or bruises on the patient and there is no video footage or witness to the allegation.

    • Client reported he fell in his room while looking out window. Injury across his left eyebrow. Nurse assessed; bleeding controlled. Sent to ER for evaluation, client denies pain. No follow-up details.

    • Client reported to nurse/supervisor that 3 staff members allowed unit to have a fight night. Client stated that 1 staff member brought boxing gloves so patients could fight one another. No injuries noted. Staff placed on leave pending investigation.

    • Patient reported that a staff member allowed peers to smoke marijuana that staff member had given to them; 3 tested positive for marijuana. Staff placed on leave pending investigation. No follow-up details.

  • Sexual Abuse
    • Client reported he woke up in his bed to find his pants down and felt something sticking in his butt. Client saw roommate hiding by his bed. Peer denies incident. Patients were moved to different rooms.
    • Child Abuse Hotline was called and accepted. It is reported that there is no visible tearing or bleeding to client. Client reported that he asked peer why he did that and peer stated that he didn’t know.

Officer was dispatched to facility at approximately 1333 hours in reference to 2 runaways. At approximately 1529 hours, Officer was advised that both Juveniles had been found.

  • Maltreatment – Staff Abuse
    • Client stated that while in a fight with peer in the cafeteria, staff member pushed him up against the wall and grabbed him by the neck. Then staff member pushed him repeatedly until he was bent over a table and pushed to the ground. 0 apparent injury; 4/10 pain at middle back. Ibuprofen and ice pack given. Staff placed on leave pending investigation.
    • Child Abuse Hotline was called, not accepted.

    • Client has a right distal stress fracture of femur. Ordered to use crutches & follow-up appt will be made with orthopedist. No complaint of pain at this time. Notations made regarding injury starting on 4/8/21.

  • Substantiated.
  • Regs out of compliance –
    • W 104 Governing Body –
      • Multiple areas of missing or chipped paint.  
      • Paneling pulled away from wall. Cabinet door missing.
      • Dark, unknown substance on bathroom counter and walls.
      • Gouge in sheetrock by bed in bedroom.
      • Air return vent damaged and bent, heating vent bent down into the duct work. Multiple vents and light fixtures had dirt and dust in them.
      • Exit sign loose with screw exposed, vent cover loose with screw exposed.
      • Multiple broken tiles on counter and floor, some with discoloration.
      • Cooking oil bin outside had buildup of black greasy substance.
      • Edging came off plexiglass window, and tape was used to keep snow out.
      • Edging/trim missing in multiple areas.
      • Towel was placed at bottom of shelf inside refrigerator due to leakage.
      • Bottom of cabinet which contained pots and pans was discolored with a black substance and had a strong, moldy smell.
      • Linoleum coming loose or missing from several areas.
      • Pipe had hole in it with aluminum foil stuffed in the hole.
      • Dishwasher had buildup of unknown brown substance.
      • Chairs/couches had multiple cracks, holes, and tears present.
      • Equipment in backyard area in disrepair with no safety signs.
      • Multiple mattresses had indentions, approximately 9 needs replacing.
    • W 159 QIDP –
      • No recent QIDP observations for several clients.
    • W 341 Nursing Services –
      • Covid 19 screening/temperatures not consistently recorded and checked.
      • Masks not consistently being worn.
    • W 371 Drug Administration –
      • Meds were administered to clients without asking questions or giving clients information related to medication.
    • W 454 Infection Control –
      • Food in freezer/refrigerator unlabeled and undated, bottles with caps missing.

Escape from Youth Service Program, Second Degree Escape

Officer received a call on 5/22/21 at approximately 1524 hours that 2 Juveniles had left the facility. On 5/23/21 at approximately 1815 hours, Officer was advised that the 2 Juveniles had been located by Jefferson County Sheriff’s Office; Jefferson County stated they would hold the Juveniles until they could be transported back to facility. Facility advised someone would be on the way to get both Juveniles.

  • Staff Abuse
    • Resident reported staff pushed her against the wall scratching her arm and attempted to pull a pillowcase out of her hands. The resident then began to punch the staff member and the staff member hit her in the eye as other staff were separating them.

Escape from a Juvenile Detention Facility, Second Degree Escape

Officer was dispatched at approximately 1021 hours in reference to 3 Juveniles that escaped the facility. Upon arrival, Staff stated they had caught 1 of the Juveniles. Officer patrolled the area and located the other 2 Juveniles, transporting them back to facility.

Second Degree Escape

Officer was dispatched at approximately 1922 hours in reference to 2 Juveniles that ran from facility towards nearby woods. Officers were unable to locate. Officer spoke with Staff on 2/27/2021 who stated that both Juveniles were found on 2/25/2021, one at 8:30pm and the other at 11:30pm.
