Population Served: Males ages 12-17 and a specialized sexual rehabilitative unit for males ages 12-17

Notice of Incident, Peer Altercation

Client was punched and kneed in the nose by a resident and hit in the face by another resident. Client complained of pain, swelling was noted to his left eye, and client’s nose was bleeding. After nurse assessment, client was sent to the ER. X-ray results: Orbital fracture of L eye. ER recommended follow-up with APRN within 2-3 days, continuance of care and follow-up with ophthalmology in 2 weeks.

Facility informed Licensing no camera footage was available of incident. Licensing Specialist was informed that resident discharged from the program and his guardian took over treatment. Peer who attacked resident was placed on unit restrictions and later discharged from the program for assaulting the resident.

Employee was heard cursing out residents (1 client in particular) in the dining hall by other employees. Employee admitted to cursing at children and resigned on the spot. Resignation was accepted and former employee will not be eligible for re-hire. The Program Director made a Child Abuse Hotline report (not accepted).

Facility cited 905.4.c: Staff member was overheard by other staff members using profanity toward the residents in the dining hall. Facility informed Licensing Specialist that professional conduct will be reviewed with all staff within 30 days. Licensing founded complaint.

Resident punched a wall when angry in the quiet room and complained of pain to his right hand. Nurse noted swelling of right hand and client was sent to the ER. Results of X-ray: None. Diagnosis: Right hand pain, contusion.

Accidental Injury, Notice of Incident

Resident complained of pain in his left hand after injuring it while playing sports/physical activity. Nurse noted swelling to his hand and client was sent to the ER. Results of X-ray: Dorsal offset of the epiphysis of the radius with suspected metaphyseal corner fracture of the posterior radios is likely a Salter two fracture. Diagnosis: Left wrist and hand trauma/contusion.

Resident punched a wall when angry and complained of pain to right hand. The nurse observed swelling to his right hand and resident was sent to the ER. Results of X-ray and physician assessment are pending.

Licensing received copy of x-ray results, no acute fracture or dislocation.

Licensing provided facility with the following Technical Assistance: 110.17. Facility did not notify Licensing by the next business day of serious occurrences that required medical treatment for 5 ELS cases. Moving forward, facility will contact Licensing via telephone when unable to email or enter serious occurrences in ELS.

Resident complained of lower back pain with shooting pains to right leg, bruise to left ear, and swelling of right ring finger. Resident expressed that he had experienced pain for the past three days and would not disclose the origin of his injuries. After nurse assessment, resident was sent to the ER. The physician did not confirm any further injuries or conditions. Instructed to take Tylenol as needed. X-Ray completed on finger and no findings present.

Maltreatment, Notice of Incident

Resident alleged that staff grabbed his neck and hurt his chest after he was guided out of a peer’s room that he had ran into. Resident and peer stated they were horseplaying and staff perceived resident running into peer’s room to fight. The actions of these two resident’s running into the resident’s room resulted in an ESI of another resident. Resident was observed hitting himself in the chest after another resident instructed him to make an old bruise “new again”. Child Abuse Hotline was called, not accepted. Safety Team conducted internal investigation, reviewed staff and resident statements, and talked with resident and 3 peers involved. The resident was moved to another hall immediately after allegations were made as to not work directly with staff he alleged hurt him.

Licensing was informed there is no camera footage. Licensing reviewed witness statements and nursing note. 10/16/2023 – Licensing Specialist inquired if staff member was working with resident. Facility reported the resident has been discharged from the program.

No licensing findings as this was not a licensing complaint investigation.

Visit Compliance Report

Licensing Specialist: Kendra Rice.

Buildings & Grounds conducted from 6:15pm – 7:00pm. Census: 56. Areas observed: Bedrooms, bathrooms, cafeteria, nurse’s stations, quiet rooms, laundry rooms, classroom hall, administration building, and gym. MARs reviewed for 5 residents. No licensing concerns noted.

Notice of Incident, Peer Altercation, Visit Compliance Report

Resident was punched in the face and attacked by another peer, causing injury to nose and causing resident to hit head on the ground. After nurse assessment, resident was sent to the ER. The ER physician confirmed head injury and conducted medical screening exam.

Licensing Specialist was informed resident involved himself in an altercation and was trying to break up a fight, he was not fighting. All parties involved were placed on assault precautions. Resident was moved to another hall for safety.

Licensing reviewed camera footage on 9/20/23. Due to the angle of the camera and the darkness of the area, licensing was unable to view the physical altercation but observed staff members running to the area to intervene. Residents were separated and were walked in two different directions of each other.

Resident reported worsening feelings of pain and flu-like symptoms with fever present. After nurse assessment, resident was sent to the ER. Client was diagnosed with an upper respiratory infection and prescribed medication.

Fire alarm. Fire Department responded and advised that everyone evacuated, nothing is showing at this time. Alarm was reset.

Resident reported that he had pain in his body after falling on 9/11/23. Resident was not able to specify why he fell. After nurse assessment, resident was sent to the ER. No findings indicated by ER physician.

Licensing Specialist inquired about late reporting: Staff member advised he was unable to log into his computer from his residence. Licensing Specialist was informed that the resident fell on the courtyard while playing catch with a football.

Caller advised dispatch that the facility would not let him have his son.

Assault

Officer responded to facility in reference to an assault. Staff advised officer that he was attacked by 2 juveniles and suffered injuries to his right shoulder, back area, and hands. It is unknown why the juveniles attacked the staff member.

Resident injured his right toes in the gym as a result of his toes being accidently stepped on. He complained of pain and was given an ice pack and Ibuprofen 200mg. APRN ordered resident to be sent to the ER for an x-ray of his foot. Results: No acute fracture or dislocation.

Notice of Incident, Peer Altercation, Visit Compliance Report

While in the dayroom, resident told his peers they were too loud and stood up and began to curse at the residents. A physical altercation ensued between client and 4 peers. Staff were able to separate the residents. Upon examination, resident was noted to have a knot under his left eye and a small knot on his forehead. Resident was sent to the ER for further evaluation and treatment. Results: Assault by unspecified means; contusion of other part of head.

Licensing was informed that residents involved were placed on assaultive precautions. Licensing reviewed camera footage on 9/12/23. Staff were observed separating the residents after the physical altercation, ratio 7:10. No licensing concerns noted.

Accidental Injury, Notice of Incident

Resident reported to staff that during a basketball game, another peer stepped down on his ankle. Resident reported he did not want to be treated because he wanted to play more basketball. Nurse assessed and observed swelling to client’s ankle. Staff explained the importance both long and short term of getting his ankle looked at. APRN recommended an ice pack be given to resident and an X-ray if further swelling and pain persists. Client placed on activity restriction.

Resident reported pain in his right hand after he punched a wall and a pole outside because he was angry. Resident was assessed and sent to the ER for x-ray. X-ray results: Fracture was indicated in hand. Pain medications PRN to be provided.

Licensing was informed that resident will have a follow-up appointment with Jonesboro Ortho on 9/25/23.

Resident reported to the nurse that he was having chest pains. Resident was assessed and sent to the ER where he was given an x-ray and medical assessments. The lungs were clear and mediastinal silhouettes were in normal limits. Resident was sent back to the facility for continued monitoring.

Resident reported to the nurse that while playing basketball, a peer “kneed him” (residents were not fighting or in conflict). Resident further stated that he fell and hit his head, and complained of head pain. Resident was sent to the ER and diagnosed with head injury. Resident was sent back to facility without any further treatment or pain reported.
