Elopement, Notice of Incident, Peer Altercation, Visit Compliance Report

Per staff report the resident eloped from the facility grounds after pulling down a smoke detector off the wall in his room causing all the facility doors to unlock.

Actions Taken: Camera review was conducted on 3/20/2023 at 12:00pm. The resident is observed standing inside of his doorway playing a game with various peers in the hall from their assigned room doorways. Staff were present actively completing quality 15-minute checks. The resident is observed going into his room and then the fire alarm went off. All residents and staff proceeded to evacuate the building following the fire alarm protocol. All residents evacuated to the courtyard and lined up on the basketball court. While on the basketball court awaiting the building’s clearance by the FCFD, the resident was in a physical fight with a peer. During the altercation, the resident took his shirt off and began fighting. Staff immediately intervened and separated the two residents fighting. Shortly after several other fights broke out while on the courtyard, staff intervened immediately and transitioned the youth back inside the facility. While transitioning back inside the resident ran past the entire group and back onto the 100-hall. The building was cleared by the FCFD at 7:47pm, but the alarm system was not online, meaning the facility doors were still unlocked. The resident ran down the hall and out the exit at 7:50pm. FCPD was called and gave the residents description. Currently the resident has not been found. FCPD was contacted again today 3/20/2023 at 1:01pm with no recent updates. The resident’s guardian has also filed a missing person’s report as of 3/20/2023 at 2:22pm. All proper notifications have been made. Licensing Specialist: Kendra Rice.

Regulations out of compliance – 907.3 – Staff/child ratio shall be at least 1:6 during waking hours and 1:8 during sleeping hours.

Accidental Injury, Notice of Incident, Visit Compliance Report

Per staff client attempted to elope from the facility by climbing the fence. While trying to climb the fence the resident received a puncture hole in each palm of hand from the top of fence. Staff immediately intervened and resident was unsuccessful. Resident was immediately taken to nursing and assessed. A referral was made by the APRN to sent the resident out to FCMC for further assessment. The resident was diagnosed with a The resident was placed on elopement precautions and unit restrictions for the next 72 hours (about 3 days). Those precautions will be reassessed for safety. All proper notifications were made, and nursing will continue to monitor the resident’s progress while in the milieu and school. Licensing Specialist: Kendra Rice.

Accidental Injury, Notice of Incident

Per staff report the resident was brought to the nurse’s station complaining Rt side hip pain. The resident stated it started hurting after playing basketball and only hurts with movement.

Actions Taken: The resident was assessed by nursing and APRN. During assessment, the resident was referred to FCMC for further evaluation and a X-ray. The resident returned from FCMC with the diagnosis of: No acute fracture or dislocation. The right femoral acetabular joint intact. Lucency through the S1 segment of the sacrum is probably a growth plate. Nursing will continue to monitor the residents’ progress while in school and the milieu.

  • Facility visited from 10am – 11:15am. Census: 58. Licensing attended SAMA training at facility to ensure staff are being trained according to facility’s policy on physical restraints, and to ensure the policy complies with the minimum licensing standards.

    • Client stated he jammed his finger playing basketball and hurt his hand/finger. Client was referred to FCMC. Diagnosis: Buckle fracture of the proximal metaphysis of the proximal and distal phalanx of the 5th digit.

  • Medical
    • Client complained of feeling dizzy, fuzzy, and sleepy. Client was referred to FCMC due to a possible adverse medication reaction. Diagnosis: Encounter for screening, unspecified. Client’s medication was discontinued by the psychiatrist on 3/3/23. Client did not return with new orders or prescriptions.

  • Facility visited from 12:00pm – 1:45pm. Census: 58. Licensing reviewed 4 resident files and 4 personnel files. No licensing concerns noted.

Notice of Incident, Self-Injury, Visit Compliance Report

  • Self-inflicted Injury
    • Client reported he was chewing on some staples found in the school and accidentally swallowed them. Client was sent to FCMC. ER Diagnosis: A staple was noted in the mid transverse colon and another in the proximal descending colon; There is no free air or evidence of obstruction. Client will continue follow-up x-rays until the staples have exited the body. As of 2/18/23, the staples descended from client’s body. Client was placed on line of sight until deemed safe by his assigned therapist.
    • Licensing follow-up: Staff reported the client found the staples in the wall from artwork that was hanging in the classroom. Teachers have been instructed to use alternative methods for hanging items. Licensing completed a walkthrough of classrooms on 02/27/23 and discussed precautions with staff. Classroom 1 had staples around the border of the bulletin board (removed by staff immediately).

  • Facility visited from 11:15 am – 12:30 pm; Census: 54. Licensing completed walkthrough and observed the following areas: Courtyard and SRU (200 Hall). A bathroom in 200 Hall had toilet paper on the floor (housekeeping corrected). MARs checked for 5 residents. All staff completed Conscious Discipline training.

    • Client was sent to the ER after complaining of stabbing stomach pains and vomiting. Diagnosis: Viral Gastroenteritis (stomach virus). Client was placed on 24-hour quarantine to prevent spreading.

At 1255 hours, staff reported a juvenile escaped the facility. Juvenile returned to the facility at 1301 hours.

At 0820 hours, the facility advised a juvenile pulled the fire alarm. At 0825 hours, the facility called back and advised a juvenile ran off the property. Officer located and transported juvenile back to facility at 0830 hours.

Caller (staff) advised a resident had swallowed glass and they are having to wrestle glass away from him. Juvenile was transported to the ER by EMS.

Officer was dispatched for an assault report. Upon arrival, staff stated that a resident became irate and tried to escape the facility. Staff stated when she tried to stop the resident, he struck her in the chest and head with a closed fist. Staff was advised of the affidavit process.

    • Client complained of abdominal pain and stated he coughed up blood; When the nurse asked to see it, client stated he flushed the toilet. Client was sent to the ER and diagnosed with abdominal pain and constipation.

Notice of Incident, Peer Altercation

  • Peer Altercation
    • Client and peer were in a physical altercation, during which peer bit client on his thumb. Client was taken to the ER and diagnosed with human bite. Client was placed on peer restriction and assault precautions.
    • Licensing follow-up: Staff reported client was sent to the ER due to complaints of pain and skin wasn’t intact. The peer involved was placed on the same precautions. No licensing concerns noted.

  • Suicide Attempt
    • Client became upset while in the gym and entered the gym courtyard, where he stated he found a small piece of glass and swallowed it. Client was taken to the ER and diagnosed with suicidal ideation. Upon return, it was noted the resident never swallowed the glass. Client was placed on suicide precautions and line of sight. Maintenance vacuumed all outside window seals to ensure no plexiglass particles were left from previous window fix.
    • Licensing follow-up: Licensing completed a buildings and grounds walkthrough on 02/15/23. No video was available due to the camera outside the gym not working; Camera is scheduled for repair. The staff member who followed the client outside stated he held the object in his mouth and never swallowed it but was sent to the ER because it could not be determined.

Notice of Incident, Peer Altercation, Visit Compliance Report

  • Peer Altercation
    • After a physical altercation in the gym, client complained of swelling/pain to his eye, and pain to his head and knee. Client was sent to the ER and diagnosed with contusion of eyelid and periocular area of eye. Both residents were placed on peer restriction, and client was placed on assault precautions for initiating the fight.
    • Licensing follow-up: Licensing reviewed camera footage on 02/08/23. Staff intervened in the physical altercation and client’s attempts to be physical with other peers.

  • Self-inflicted Injury
    • Client became upset with a peer and punched a window, shattering the glass in the gym courtyard. Client was sent to the ER; No acute fracture or dislocation. Client was placed on self-harm precautions. The glass of cleaned up by maintenance and the window was replaced on 02/03/23.
    • Licensing follow-up: Licensing confirmed the client was assessed by nursing before being sent to the ER and was informed the window was replaced with plexiglass.
