- Licensing (Self-inflicted Injury)
- Client complained of head pain from a self-inflicted injury and left posturer neck pain from hitting his head on the fence. Client was taken to the ER. Client was placed on self-harm precautions and a safety plan.
- Licensing follow-up: Specialist observed client upset due to a room change, and client became aggressive with staff. Client was placed in a restraint, and once released, client began to hit his head on the wall of the SRU hallway. He then walked by the door and began to hit, kick, and bang his head on the door, during which staff appeared to be talking with him. No licensing concerns noted.
