Population Served: Males ages 12-17 and a specialized sexual rehabilitative unit for males ages 12-17

Staff Abuse

  • Resident was slammed to the ground and unnecessarily restrained by staff member. Staff member terminated. Incident identified after guardian called facility and asked for MRI of resident.
  • Child Abuse Hotline was called and accepted.
  • Agency was cited on regulations 905.9, 905.10, 110.12, and 110.17.

  • Reviewed 2 personnel records and completed walkthrough of building and grounds.
  • Regs out of compliance –
    • R911.6 – Repair or tighten screens for the air conditioner in rooms 207, 406, 407, and 303. Correction date 12/11/20.

Staff reported individual at facility trespassing and needs to be removed. After officer arrived on scene, individual left the location.

Staff reported 2 Residents ran away. 1 Resident was recovered and brought back to facility, reporting it was two other Residents that left with recovered Resident.

  • Regs not correctable –
    • R103.8 – From documentation received from staff on 9/24/20 via email, staff was hired on 8/31/20 but background checks were not initiated until 9/24/20.

Staff reported that 2 Residents had run from the facility and jumped the fence. Staff reported deputies had returned both Residents to the facility.

Visit Compliance Report

  • No regs noted out of compliance.
  • Staff showed Specialist the fence outside due to the multiple elopements at facility. Staff stated they plan to put slates in the fence so the residents cannot get their feet in the crevices of the fence to climb over. Staff emailed Specialist copies of Medication Records, Observation Records, Master Insurance Certificate, Vehicle Insurance Cards, Fire Drills, and AR Health Inspection License.
  • Specialist also discussed with Staff Covid-19 concerns; Staff stated they currently have 41 residents that tested positive, and they have been separated from the other clients. Staff stated they continue to remind kids to wear masks, but some are defiant. Only residents that test negative are required to go to school.

Staff reported Resident left facility walking towards the nursing home across the street, Resident positive with coronavirus.

Staff reported Resident left the facility out of the back door. Staff called back to report the Resident had returned.

Staff reported that 2 Residents had run from the facility and jumped the fence. The 2 were later escorted back to the facility by deputy.

4 Residents took AC Unit out of window and left the facility through the window in room. All juveniles had coronavirus. Staff later advised that all juveniles were back at the facility shortly after one of the Residents had been escorted back by officers.

Information Report

Staff reported that Resident who eloped on 7/23/2020 (recovered same day) broke into the nursing home across the street from the facility by throwing a rock into the window and crawling through. Resident stated they tried to get into the main building but were unable to do so. Nothing was taken, and no charges are being pressed.

The same 4 Juveniles that had ran away 7/25/2020 ran away again. All 4 entered in ACIC as missing juveniles.

Caller reported 3 Juveniles ran away from facility. Juveniles had coronavirus and were in an isolated room at the facility. In the following report dated 7/26/2020 4 Juveniles had been recorded. All were recovered in the same day.
