10/30/2023 – Licensing Specialist informed there was no camera footage and peer was placed on assault precautions.

Buildings and grounds completed from 10:30am – 12:45pm. Areas observed: Grounds, dining room, administration building, nurse’s station, bedrooms, bathrooms, parking lot, and courtyard. Ratios observed during walkthrough. MARs reviewed for 4 residents. No licensing concerns noted.
Record Review conducted from 12:45pm – 1:30pm.
Licensing reviewed 4 children files and 3 personnel files with no licensing concerns noted. Licensing received documentation of a staff member’s suspension.

Deficiencies were noted during the Inpatient Psychiatric Inspection of Care (IOC) conducted on 10/17/23.

Battery
Officer was dispatched in reference to a resident assaulting a staff member.
Upon arrival, the resident advised officer that he was told he had to move halls and he wasn’t moving as fast as the staff member wanted him to, so staff started to throw his things and he got upset. Resident advised officer the staff had grabbed him and he defended himself, stating he complied with all staff orders and the staff member was aggressive with him.
The staff member advised officer that the juvenile was moving at his own pace and became aggressive. Staff stated he attempted to put the resident in a restraint, and he assaulted him.
Both the resident and staff member had red marks to their nose and scratches to their necks. Facility was advised of the affidavit process.

In the classroom a desk fell on resident’s right foot causing injury. Nurse note pain and swelling of the foot. Resident was sent to Forrest City Medical Center for evaluation. Diagnosis: Foot contusion. No significant findings on x-ray.

Resident received laceration to the eye during an accident that took place while playing with a peer during recreation time. Nurse observed the skin above the eye broken. Resident was sent to Forrest City Medical Center for evaluation. After the medical exam there were no other findings other than the eye laceration and some swelling.

X-ray findings: The cardiac and mediastinal silhouettes are within normal limits. The lungs are clear. The osseous structures are intact; No acute cardiopulmonary abnormality.

Resident took off splint and threw it away. Resident was sent to Forrest City Medical Center for another assessment, who then referred resident to Le Bonheur Hospital for cast reevaluation and hard cast to be put on it to reset fracture and immobilize it so the wrist can heal properly.

Battery
Officer was dispatched to Forrest City Medical Center for a battery report. Upon arrival, juvenile advised officer that he was lying in bed at the facility when a peer walked over and hit him in the eye with a closed fist. Juvenile was then sent to the hospital and diagnosed with a fractured left lamina papyracea. Officer made contact with staff who advised they would make contact with juvenile’s guardian to sign an affidavit to press charges.
