Caller stating Juvenile was becoming violent after being discharged from facility since no bed was available; Juvenile getting violent with staff. Upon Officer arrival, Staff stated no police action was needed and that they would be conducting a transfer themselves.

  • Maltreatment – Staff Abuse Allegation
    • Guardian called hotline to report staff member “grabbing” clients arm and causing a bruise. After completing internal investigation, it was not believed there was any wrongdoing from the staff member. Staff member will not work without direct supervision and will not be allowed to provide any discipline.
    • Child Abuse Hotline was called and accepted, unsubstantiated.

    • Peer suffered a very swollen right eye and cheek after being hit by peer. Diagnosed with closed fracture of the nasal bone.

  • Piney Ridge Treatment Center has failed to maintain substantial compliance with the Minimum Licensing Standards in the areas of buildings, grounds, and behavior guidance.
  • The compliance history listed reflects the excessive citations and lack of sustained compliance at Piney Ridge Treatment Center. Because of this, youth are not receiving the required care as intended by the Minimum Licensing Requirements.

  • Viewed all units, gym, classrooms, cafeteria, and yard with no safety concerns. Viewed annex – being used as emergency placement for 13 boys from Ridgeview Group Home since a pipe burst there. Viewed med logs, no concerns. Census 86 with admit expected later tonight. All areas in ratio.

    • R911.6 – south hallway has scuff marks from a child’s boots all over the walls. Trash in gym bathroom was overfull. Trash changed on site; wall has work order.
    • R913.5 – pillow had stuffing coming out – replaced onsite.

Juvenile got upset with Staff because he was asked to turn out his pockets; Juvenile punched Staff in face. Juvenile was walk in, Juvenile was in seclusion room receiving medications upon Officers arrival.

  • Viewed North, West, South, and East units, gym, cafeteria, and yard.
  • Cafeteria has been cleaned. Jan Pro is scheduled to deep clean cafeteria and bathrooms. Bedrooms were tidier and reward and chore charts are being utilized. Materials still unavailable to enclose outside AC unit. Rooms are in process of being painted.
  • Regs out of Compliance:
    • R1003.11 – ensure diagnosis included on the treatment plan. Corrected 2/2
    • R911.15f – Ensure clothing items with drawstrings are not allowed in rooms. Found in multiple rooms. Corrected on-site.
    • R1012.6 – Ensure paper towels and soap are available. Corrected on-site.
    • R1013.6 – Ensure mattress fits bed, mattress too small for bed frame. Corrected on site.
    • R1012.4 – Ensure shower does not flood floor in 301. Corrected date not listed, only “extension requested” past 02/21/21.

Licensing Complaint

  • Complaint that resident is being discharged in retaliation for complaints guardians have made about PRTC. Complaint unfounded.
  • Licensing specialist visited facility on 01/14/21, no specific discharge date listed for resident and risk manager stated child would “continue in treatment until he finished the program that was deemed necessary for him to complete.”

  • Resident reported he punched a bed frame the night before. Hand was swollen and an x-ray ordered. X-ray revealed an acute nondisplaced transverse fracture mid 4th metacarpal. Ordered to see Ozark Orthopedics.

      • Classroom 103 – only one teacher has key.
