  • Unannounced visit was conducted at main campus and annex building. Annex building is being used for quarantine purposes only. This building is not psychiatric safe, and children shall be within eyesight at all times, when not in restroom.
  • R1011.7 – ensure hole near volleyball area is filled, air conditioning units are fenced. ANNEX ensure hanging wire by front door is repaired, emergency lights hanging by wires are repaired, lock recommended for open electric box by exit door, replace light switch, ensure chemicals are not stored in children’s rooms, ensure wall and ceiling are cleaned near trash container in children’s rooms, floors shall be vacuumed, replace light fixture covers in ceiling.
  • R1008.3 – ensure noon meds are logged as dispensed, CD 11/6/20.
  • R1012.4 – ensure toilet in common area in west is cleaned and lid replaced, boy’s toilet in annex is repaired, CD 11/24
  • R1014.6 – ensure evacuation map is posted in annex building. CD 11/7
  • R911.15 – ensure draw string shorts/pants and show laces are not available to children. CD 11/5

      • Failed to ensure there was an active order for use of restraint and/or seclusion procedure for client.
  • Plan of Correction: The Director of Nursing reviewed resident records and facility policies and provided training to all nurses. Random audits are also completed.

Stolen Electronics

Caller advised of stolen iPhone 11, and that the nurse on shift at the time that it was stolen stated that video footage was not viewable due to it being live feed. Caller stated she didn’t know if nurse was telling the truth or didn’t want to bother helping her.

  • Specialist and Assistant Director conducted unannounced monitoring visit, viewing all areas of facility. After completion of walkthrough, debriefing was held with CEO. Several residents were not wearing face masks during this time of pandemic.

    • R912.4 – Wet tissue on floors, dirty laundry on floors on East unit rooms 305 and 306 and North unit rooms 204 and 206 (Correction date 8/17/20).
