Location: Little Rock, AR

Population Served: Adolescents 12-17 years of age in need of mental health services on a 24 hour a day basis.

# of residents per unit: 12 Residents per room: 1-2 Capacity: 54

Contact with family (Calls and visit schedule): Kids have daily contact with their family either via zoom, face time calls, or phone calls. In the evening they are given time to call families and their families are welcome to call anytime during the day. Outside of Covid-19 Restrictions families can visit on the weekends and once during the week.

Notice of Incident

  • ER Visit (Allergic Reaction)
    • Client presented to nurse with labored breathing due to possible food allergy. Client stated she felt funny with a lump in her chest. Client received EPI pen and was transported to the ER. Client returned from the ER with no new orders.
    • Licensing follow-up on 4/28/22: It is noted in the chart that client is allergic to shellfish, seeds, and nuts.

  • Self-Harm
    • Client reported to staff that she used string from a Covid mask to cut into her neck area. Client had a barely visible red mark (about 4 inches) on the supine side of neck. Client denied thoughts of self-harm currently. Orders were received for increased observation status. No follow-up details.

  • ER Visit (Enlarged Lacrimal Gland)
    • Client complained of severe eye pain, and nurse noted lacrimal gland protruding and clear fluid leaking out. Client was taken to the ER. Client returned from ER in stable condition and was prescribed eye drops.
    • Licensing follow-up on 4/28/22: Specialist sent email to facility to check the status of client. Facility reported a follow-up appointment with the eye doctor on 4/26, however the consult sheet from that visit has not been turned in to medical records. A consult sheet was sent via email which states that client had enlarged lacrimal gland. Prescription eye drops ordered.

  • Urgent Care Visit (Self-inflicted Injury)
    • Client complained of hand pain after reportedly hitting the wall sometime the week before. During medication checks on 4/15/22, staff decided it did not warrant an Urgent Care visit. This week, client reportedly hit the wall again. Client was taken to Urgent Care due to swelling. Client was diagnosed with a right-hand sprain; RICE treatment prescribed, and Ibuprofen/Tylenol as needed for pain. No follow-up details.

Runaway Juvenile

Staff stated while outside for recreation at around 1530 hours, 2 juveniles ran away and did not return (officer dispatched at approximately 1656 hours). Staff stated he went looking for them but could not locate them. On 4/17 at around 0012 hours, officer responded to a welfare concern and located both juveniles. Officers returned juveniles to facility where MEMS checked them for injuries (none).

  • Elopement
    • During a walk with staff, 2 clients ran into the woods. Both clients returned, 1 of them “via sheriff’s office and ambulance”. Client was treated by nurse for minor scratches. Report states that after elopement, staff returned to the house and followed AWOL procedures.

  • ER Visit (Accidental Injury)
    • Client fell while walking down the outside stairs, hurting her ankle. Client was transferred to the ER. Client returned from ER with recommendations to wear boot and use crutches. No follow-up details.

  • Suicidal
    • Client voiced that she was suicidal. After staff informed client she would be placed on “eyeball” (suicide precaution), client reached for leggings so she could strangle herself.
    • Licensing follow-up on 4/14/22: When asked, facility indicated that they lost power on 4/13/22 due to weather and were unable to send report timely.

  • Urgent Care Visit (Spider Bites)
    • Client complained of itching red rashes on her lower left leg during day shift. Nurses evaluated and suspected it may be spider bites. Client had reportedly found a dead spider in her room. Client was taken to Urgent Care to have her leg evaluated. No follow-up details.

Terroristic Threatening Second Degree

QRTP – Staff stated juvenile refused to return remote control (due to watching inappropriate videos) and threatened to spit in staff’s face and hit him in the head with a rock. Juvenile then knocked staff’s hat off his head. Juvenile was cited for Terroristic Threatening 2nd.

Visit Compliance Report

  • Licensing Specialist: Clayton DeBoer.
  • Buildings and grounds conducted from 11:00am – 1:00pm and MARs checked for 5 clients.
  • TA –
    • R911.15.f – One bedroom had two pairs of shorts that had strings in the waist long enough to inflict harm to a client. Staff consulted whom advised that house staff would inspect all rooms for overlooked strings in bedrooms.

QRTP – Staff stated juvenile had received a discipline for bad behavior and became angry and began to physically fight staff, stating juvenile punched, kicked, scratched, and tried to bite staff. Officer observed multiple scratches, marks, and swelling to staff. Juvenile was arrested & issued citation for 3 counts of Battery 2nd, then released back to custody of staff.

Staff stated juvenile got upset because she wanted McDonalds, and acted as if she was going to fight staff. Client then went outside and started to throw rocks at staff’s vehicle and kicked the back bumper. Juvenile’s clinical therapist requested MEMS assess juvenile as her behavior has been alarming. Juvenile was transported to ACH for further assessment.

Staff stated juvenile came home from school upset, and began to tear things up and cause a disturbance. After an attempted restraint, juvenile struck a staff member (who wants to press charges). No injuries reported. Juvenile was issued citation.

  • Facility visited from 12:00pm – 2:00pm. Staff records reviewed with no licensing concerns. No new hires or concerns with background checks reported by HR at this time.

Staff stated juvenile got upset because he thought he was grounded. Staff stated after entering medical room, juvenile entered the room (not permitted to do so) and resisted when staff attempted to remove him. Juvenile bit staff member on her hand, officer confirmed bite marks. Juvenile was cited and officer collected witness statements/photographs.

Staff stated juvenile got upset because he couldn’t get on YouTube and threw chair at staff member. Staff stated a peer handed a game to juvenile and sucker punched him in the face. Juvenile then picked up a broom and began swinging it in an attempt to hit peer, but hit staff in the process. No injuries reported. Juvenile was issued citation.

  • Suicidal/Threatening Behavior
    • Client was upset about peers no longer communicating with her since she came back from acute and wrapped her leggings around her throat in an attempt to harm herself. PRN Hydroxyzine administered per request at approximately 6:30pm. Client placed on FOH/Eyeball, further upsetting client. Client attempted to tie clothing around her neck, requiring restraint. After an upsetting call with her guardian, client tried to tie clothing around her neck again, requiring another restraint. The next morning (3/16), client expressed being suicidal to staff and tried to tie shirt and jeans around her neck. Items were taken and client placed in a restraint. Client processed with staff and remains on eyeball.
    • Specialist followed up on 3/18: Client was placed on front of house, supportive status, eyeball, and restricted status. Client continues to reside at facility with no further incidents as of 3/18.

Complaint #AR00027902 was substantiated, all or in part, without deficiency. Complaint #AR00027918 was unsubstantiated.

Suicide Threat

Officer was dispatched to assist MEMS with juvenile that had threatened suicide. Staff stated juvenile had been acting out and had to be restrained a total of 7 times between 0830 and 1208 hours. Staff stated at one point juvenile placed a sheet around his body and said “I’m going to kill myself”. Juvenile was calm on his bed and complied when staff told him to get up. Juvenile was transported to Methodist Behavioral Hospital in Maumelle.

  • Suicidal/Threatening Behavior
    • Client became upset after breaking a watch which staff took and indicated they would try to fix. Client screamed and started flailing around on the ground. Client was placed in seclusion and indicated he wanted to kill himself, putting his hands around his throat as to choke himself. Client continued to fight staff at which point a chemical restraint was implemented, during which client punched staff in the face multiple times and spit on staff. Client was placed on eyeball and remains at facility.
    • Specialist followed up on 3/8 and reviewed video of incident. No licensing concerns or improper holds viewed during seclusion, restraint, or chemical restraint.

  • Suicidal/Threatening Behavior
    • Client had been upset and processed with 4 different staff regarding the issue. Client tried to choke herself in which staff intervened. Nurse assessment: Client complained of slight pain to neck area. Staff debriefed with client about what happened and what caused her to try and choke herself. No follow-up details.

  • ER Visit (Self-inflicted Injury)
    • Client became angry due to receiving a consequence for being sexually inappropriate. Client went to her room and punched the wall, causing immediate swelling/bruising to the left 5th digit knuckle. Client was given Ibuprofen and ice pack. FOH and eyeball status ordered. Client taken to Urgent Care (3/4) for x-ray. Client’s hands were bruised and swollen, no fractures; Ice and Tylenol recommended.
    • Follow-up: Urgent Care Note – No fractures to either hands contusion/bruising of both hands. Recommendations: ice hands. Can wear ace wraps for comfort. Take over the counter Tylenol/Ibuprofen.

  • Urgent Care (Accidental Injury)
    • Client was in a hurry going back into game room and tripped on the bumper between the gym and game room, spraining her ankle. Client was sent to Urgent Care for x-ray.

Battery Second Degree

Staff stated J1 became disrespectful/upset while in group therapy and was kicked out. J1 began cursing at staff and swinging a chain around, in which he hit staff on the forehead and lip. Staff restrained J1 until he calmed down. J1 charged with Battery Second Degree. While on scene, J2 started acting out, threatening to kill staff, and spit on staff. Pulaski County Juvenile Intake approved the intake of both juveniles. J2 charged with Terroristic Threatening 1st Degree. Both taken to JDC.
