Location: Little Rock, AR

Population Served: Adolescents 12-17 years of age in need of mental health services on a 24 hour a day basis.

# of residents per unit: 12 Residents per room: 1-2 Capacity: 54

Contact with family (Calls and visit schedule): Kids have daily contact with their family either via zoom, face time calls, or phone calls. In the evening they are given time to call families and their families are welcome to call anytime during the day. Outside of Covid-19 Restrictions families can visit on the weekends and once during the week.

Therapists: 8 full-time therapists (4 LAC, 3 LCSW, 1 LPE-1); 1 part-time/contract therapists (1 LCSW)

Incident Report

  • Resident began to choke himself while banging his head on the wall. Went to room to calm down, staff followed, voiced plan to strangle himself with hoodie. Observed with some erythema to the anterior neck. Resident expressed sadness for behaviors that led to placement. Placed on eyeball safety precaution – this led to further escalation. Continued to threaten to hurt himself, began banging head on wall – lead to personal restraint.
    • Injuries: erythema to anterior neck, headache from banging head.
  • PRLU F/U: licensing has requested to view video footage during the time of the report.

PRLU System Information

  • Resident flipped over chair, yelled explicit and racial slurs and began throwing his personal belongings into milieu after being told he would receive a consequence. “Placed in restraints and seclusion room due to his aggression toward staff and peers.” Observed attempting to choke himself while in seclusion room. Yelled and cursed when asked to stop. Staff entered seclusion room and removed shirt.
  • Explanation of no interim corrective action: staff has followed proper protocol by taking the proper measures to redirect, de-escalate the situation. Specialist will f/u

Visit Compliance Report

  • Viewed Sturgis and 3 rooms in Chestnut. Hickory Home viewed. Made aware during visit a therapist tested positive for Covid-19 and several kids were being put on immediate quarantine. “As we tried to visit Mabee and Rose House, we were made aware that…clients from those homes were also being put on immediate quarantine. Chestnut, Sturgis, and Mabee homes were in ratio.” NOTE: says Mabee in ratio but also did not go to Mabee.
  • Regs out of compliance:
    • R911.15f – drawstrings found in multiple houses. Corrected on-site.
    • R912.4 – ensure gym bathroom is clean. Corrected on-site.
    • R911.6 – baseboard, wall, and door lock repairs needed. No correction date.
    • R409.6 – Ensure chemicals are secure, located in kitchen and laundry. Corrected on-site.
  • TA –
    • R908.5 – Nurses and Maintenance have key access but are not in the buildings at all times.

PRLU Incident Report

  • Resident restrained and put in seclusion room. Resident bit staff. Resident injury: bump on arm and leg. Long narrative on report.
  • Interim Corrective Action: S followed proper protocol, was seen by nurse to check bump on arm and leg.

Battery Third Degree

Staff stated Resident had become upset when another peer had asked for some space. Staff stated that after the Resident had started to throw things and punched the television, Staff restrained the resident and walked them to the seclusion room. Resident then broke loose and hit Staff in the head, and then tried to wrap headphones around Staff’s neck. Staff stated he suffered injuries resulting in a hospital visit.

Aggravated Assault

Upon arrival, Deputies found Resident holding a sharp piece of wood and also one with nails in it. When approached, resident raised these weapons as if to throw them at the Deputies. After several verbal commands, Resident was handcuffed. After attempting to lunge at staff and retreive the Deputies weapon, resident was place in the patrol vehicle. Staff stated the resident had become angry and violent after candy had been taken away. Resident was transported to the Juvenile Detention Facility.

Criminal Mischief

Staff stated that Resident had become upset about post staff status, and began destroying property. Staff stated Resident calmed down after, and resident wanted to call lawyer stating they no longer wanted to be at facility. Staff stated they would like to think about pressing charges for property destruction.

Medical Emergency

Staff advised Deputies that Resident needed to be transported to Pinnacle Pointe due to an issue that happened earlier in the day, and stated the Resident has a history of being violent. After speaking with the Deputy, the Resident was transported by MEMS without issue.

  • Maltreatment Complaint received May 2020.
  • Interim Corrective Action: Staff suspended from work pending investigation outcome.
  • Findings: Maltreatment report founded 6/2020. Licensing report founded 8/2020 based on regulation.
    • 405.5 – Child caring staff shall be responsible for providing the level of supervision, care, and treatment necessary to ensure the safety and well-being of each child at the facility, taking into account the child’s age, individual differences and abilities surrounding circumstances, hazards and risks.
  • Final Corrective Action: Staff no longer employed at facility.

  • Maltreatment/Licensing received 7/31/20.
  • Interim Corrective Action: Staff suspended from working pending investigation. Staff will be terminated upon true finding based on video.
  • Findings: Maltreatment report founded 8/2020. Licensing report founded 8/2020 based on regulations.
    • 404.2 – Discipline shall be directed toward teaching the child acceptable behavior and self-control.
    • 404.3 – Discipline shall be appropriate for the child’s age, development, and history.
    • 404.4h – Physical injury or threat of bodily harm.
    • 404.6 – Physical restraint shall be performed using minimal force and time necessary.
  • Final Corrective Action: Staff was terminated by agency during investigation process.

Runaway

Staff stated that Resident had attempted to runaway from the facility. Resident was later found by staff walking on the roadway and was escorted back and assessed for injuries. Resident was instructed to take medication while Deputies were on scene.

  • Interim Corrective Action: Staff suspended from work pending investigation outcome.
  • Findings: Maltreatment report unfounded 7/2020. Licensing report unfounded. ICA can be lifted, and staff can return to normal duties.

Resident stated that a staff member put their hands on the residents throat back on 2/28/2020 in response to a disturbance between 2 residents. The resident advised a staff member of the incident, and photos had been taken of their neck. After the deputy spoke with the staff member, she stated the resident was upset with her today because she got the resident in trouble for spitting and they were separated to calm down. The Resident stated they just wanted to make a report of what happened in February. Incident number was provided to both staff and resident.

Deputy arrived at Children’s Hospital in reference to battery. Resident stated that another resident had come into room at Youth Home and struck them in the face “at least 20 times”; Resident had a broken nose and believe to have been knocked unconscious. PCSO was never contacted by Youth Home Staff in regards to this incident. Deputy provided report, and will follow up with youth staff at a later time to obtain further information.

Staff stated that Resident was being disrespectful and disruptive, and after Resident started eating a crayon Staff attempted to stop her. Resident slapped Staff on the arm. After help of other staff, Resident continued to kick, slap, and pull Staff’s hair. Staff advised they would like to press charges. An ADR was created and given to Staff.

Information

Deputy contacted Staff by phone; Staff stated after Resident was asked to go to bed, Resident kicked Staff. Staff stated Resident has been combative toward staff members resulting in being punched, spit on, and verbally attacked. Staff stated no medical attention was needed.

Staff stated that Resident had run off the property, but returned prior to the Deputy’s arrival. Resident was safe and speaking to another staff member. Staff was advised of the report.

Officers secured the scene for MEMS to transfer Resident to Methodist Hospital without incident.

When the Deputy arrived on scene, Staff stated the Resident had returned and was now very combative and disruptive. Resident was uncooperative in language but not physical at the time. MEMS transported Resident to Children”s Hospital for mental health evaluation. Resident had runaway because of an altercation between another resident, in which that resident had been treated for injuries.

Staff wanted to file a report because a Resident had been attacked a few days earlier. At the time, the Resident stated they were slapped and punched by another resident, and nothing was said or done after advising staff. The Resident did not have any injuries and refused medical attention. The report number was given to staff and the resident.

After resident refused to eat breakfast in their room, the resident retreived a plastic knife from the kitchen at unknown time. After staff escorted resident to their room, the resident lunged at staff with knife. Another staff member was able to knock the knife out of the residents hand. MEMS responded and transported Resident to Children’s Hospital for treatment.

Staff stated that 2 residents had swallowed batteries. Both were transported by MEMS to Children’s Hospital for treatment.

Staff stated that Resident struck them and 2 other staff members when asked to change clothes. All 3 staff members advised they were not injured and did not require medical attention.

Officers secured the scene for MEMS to transport resident to another location without incident.

Runaway

Staff reported that resident had caused a disturbance at the facility, stole a bike, and fled into the woods on foot. A search was conducted of the area with negative results. Resident was entered into ACIC/NCIC as a runaway.
