Contact with family (Calls and visit schedule): Kids have daily contact with their family either via zoom, face time calls, or phone calls. In the evening they are given time to call families and their families are welcome to call anytime during the day. Outside of Covid-19 Restrictions families can visit on the weekends and once during the week.

Resident became argumentative and struck Staff on the face. Deputy spoke with Resident about altercation and explained the consequences.

After verbal altercation between residents, staff evacuated the classroom so resident could be restrained. After evacuating, resident threatened other resident. After deputy spoke with resident, resident stated they didn’t mean what was said.

Staff stated resident was throwing rocks at staff members and kicking doors. The resident was transported to Rivendell Behavior Health Services of Arkansas without incident.

Resident intentionally cut themselves with a razor blade. Staff removed blade from resident but had no details of injury. MEMS arrived and transferred resident to AR Children’s Hospital for both physical and mental evaluation.

Staff stated resident threatened to “put a bullet” in another resident. Resident apologized for the comment.

Resident stated that after verbal exchange between another resident, they were slapped and punched in the forehead sustaining a minor injury.

Staff stated resident ran away while outside speaking with staff; resident returned while deputies on site, then ran out backdoor again, Resident was located in a field near the youth home. After speaking with deputies, resident returned and went inside with staff.

Staff stated that 2 residents were involved in a physical altercation. When Staff assisted with the altercation, Resident slapped Staff in face. Staff received medical evaluation. Deputy was advised to cite Resident out to Juvenile Court and issued a court date of 2/11/2020 for the Resident.

Day Treatment – Resident made verbal threats to “stab” and “drown” another resident; Resident making the threats was given two days of suspension at home.

Deputy made phone contact with staff; Staff stated that a Resident was being disciplined for instigating a situation with peers and became upset after asked to write an essay, and attempted to run. After kicking staff and attempting to throw and cut staff, other staff assisted and resident was placed in an acute unit for 2 days.

Officers responded to assist MEMS with transporting Resident to Pinnacle Point. Resident was transported without incident.

Staff stated that while at DHS office, 2 Residents were arguing about who would ride in the front seat of van. Juvenile 1 pulled Resident from the van, body slammed him to the ground and started kicking him in the face/arm area. Staff separated the 2; After returning to facility Juvenile 1 still being verbally aggressive. Resident received minor injuries and was seen by medical staff at facility; Officer took photos of injuries.

Staff stated a Juvenile had cut on his arms the previous evening (11/6/19) at unknown time. Juvenile stated his mother found out he was gay, and stated he would rather kill himself than go home with his mother. Pinnacle Point was contacted and they informed Staff that Juvenile would need to be checked by Children’s Hospital before being transported to Pinnacle Point; Juvenile was transported to Children’s Hospital without issue.

Staff stated a day student arrived to school in a bad mood and very angry; Student stated upon entering classroom “I hope everyone in the room dies a slow and painful death” and also stated “I will bring a grenade to school and blow everyone up.” Student also threatened another student. Juvenile Intake was informed.

Staff stated a Student 1 (Day student) gave another student a pill to take; Student 2 informed Staff that she had taken the pill. Student1 stated that it was not a narcotic that he gave Student 2, but his prescription medication used for depression. Student 2 appeared to be normal, no MEMS was called. Student 2 sent back to class; Student 1 was expelled and taken home by grandmother.

Caller (parent) informed Officer that she received a call from a student at the facility stating he would “beat the shit out of [child/student]” and “kick [child/student] ass and put it on GD”. Caller requested the report be made in case any incidents occur at facility.

Staff stated Juveniles 1&2 (Day students) were making threats against Juvenile 3 at facility concerning a social media post by Juvenile 3. Juveniles 1&2 stated they would “Shoot up [Juvenile 3s] house as well as the whole street” and “this would be taken care of by GDs (gang)”. Staff suspended both juveniles from school the next day.

Staff stated they found a bag of marijuana in a stuffed bear found in a room shared by 2 students. Deputies took possession of marijuana.

Staff found Student unconscious, lying face down in her closet with a shoe string tied around her neck; Staff cut shoe string while calling 911, and a few minutes later Student gained consciousness. Student was transported to Children’s Hospital.

Caller (parent) advised she believes her daughter (student) at facility is being abused and mistreated by staff. Caller stated Student had called her stating she got bored (on 7/19/19) and created a slip and slide in the hallway at facility. Student stated that Staff 1 had grabbed her hands, and after Student resisted Staff 1 grabbed her by the throat picking her up off the floor and threw her in a room causing her to bump her head. Staff 2 stated that Student became out of control when Staff 1 attempted to restrain her after becoming upset about the game stopping – Staff 2 did say she noticed Students feet off the ground but did not see hands around her throat. Staff 1 was not available to give statement. Staff stated camera system was not working that day. Copy of report was forwarded to Child Abuse Hotline.
