Officers responded to an unknown trouble at the listed location in reference to a 911 hang up. Upon arrival, officers made contact with Scott who stated someone called on accident trying to use the phone improperly. Scott advised officers were not needed.

Client complained of chest pain and headaches, then went to the nurse’s station because of a nosebleed. The client’s nose was no longer bleeding when the nurse assessed it. The client came back later during the day with a second nosebleed and the nurse observed a blood clot on the tissue. The client was also still complaining of a headache and chest pain. The client was then sent to Arkansas Children’s Hospital and returned with an order for nasal spray. Facility reported client was diagnosed with Epistaxis.

Client was transported to Arkansas Children’s Hospital (ACH) after an X-Ray was completed on March 9, 2024 due to complaints of knee pain stemming from an incident that occurred on March 8, 2024 where the client was dysregulated and kicking the doors in the dorm. Client was seen by staff at ACH and diagnosed with “Nondisplaced fracture of right tibial spine, initial encounter for closed fracture.” Client was given medication and will have a future follow up appointment with ACH Orthopedic clinic.

Program Coordinator visited facility on 3/11/24 and reviewed camera footage and requested nursing notes. On 3/14/24, Program Coordinator reviewed more camera footage and amended 521.
During video review, the resident was observed going back and forth to the corner (kicking the door). Program Coordinator observed resident communicating to a staff member as she limped around the day area. Resident eventually limped to the door leading off the dorm and sat on the floor by a peer where a staff member was standing. Program Coordinator observed peers tending to the injured resident, but did not observe staff members present assess the injured resident. From the camera footage, it appeared that the resident’s right leg below the knee was swollen. During med pass, the nurse assessed the resident’s leg, and an ice pack was provided.
Facility is being cited 907.2 – staff did not provide level of treatment to resident after she was observed limping, and 907.3 – being out of ratio 1:8 during waking hours.

Client was in a physical altercation with another peer and was bit on the left forearm by the other peer. The bite mark became bruised and was tender to the touch. Client was transported to Arkansas Children’s Hospital for an evaluation. The resident was given antibiotics at ACH.
Facility informed Licensing that the peer was placed on assaultive precautions, and the ratio at time of incident was 1:4. The police were not called.

Client was accepted and transported to Bridgeway Acute after verbalizing that she had suicidal and homicidal ideations.

Client was taking a shower and the staff was standing in the bathroom doorway following line of sight precautions and verbally checking with client every few minutes. Following a few prompts from staff verbally checking on the client, she did not respond. Staff entered the shower and found her sitting on the floor with an article of clothing tied around her neck. Staff removed the clothing from her neck, and she was responsive to them.
Facility reported clothing was pants. Facility also reported that resident was assessed by the nurse and all vitals were within normal limits. Resident was already on Non Suicidal Self Harm B, Line of Sight, and Bathroom Protocols. Facility is in the process of transferring the client to acute care. 3/1/2024, client transferred to acute care.

Client was transported to Arkansas Children’s Hospital for stomach pain. An x-ray was performed and she was given an enema. Hospital staff recommended that the client take Miralax over the weekend for constipation.

Program Coordinator inquired about camera footage and property damage. 2/28/2024, facility reported the police were called and damaged property had been repaired. Facility reported no pictures were taken of damaged property except for the door. Items that were broken were air vents, emergency lights, toilet paper holders in the restrooms, and a magnetic lock on the back door of dorm 2. Client will not be returning to the facility. 3/13/2024, LRPD reported no police report.

Building and grounds were completed. The following areas were observed: administrative building, dining room, nurse’s station, classrooms, bedrooms, therapists’ hall, bathrooms, day area, and lobby.
EMAC – Classrooms: (1) ratio 3:9, (2) ratio 3:8, and (3) ratio 2:8. Residents were sitting in their seats or talking to staff. All classrooms were preparing for lunch. Bedrooms were organized on dorms 1, 2, 3, and 4. No more than three residents to a room. Bathrooms were clean and sanitized.
EMCC – Residents were in the dining area having lunch. No residents were present in the dorms. A staff member was straightening up the day area in the East Dorm. No one present in the North Dorm. Bedrooms and bathrooms were clean and sanitized. South Dorm was being cleaned by housekeeping. No one present on New Dorm, and it was clean and organized.
Due to nurse preparing for med pass; Program Coordinator did not review MARs.

Client became dysregulated and threw water on a peer. Another peer then physically attacked the client, hitting him in the head multiple times. Staff intervened and separated the two. Client was seen by a nurse and the nurse noted significant swelling to the face and also bruising. While being examined by the nurse, the client began banging his head into the wall and being aggressive toward himself. He was transported by staff to Arkansas Children’s Hospital, where he was seen by a doctor for a head injury and given ibuprofen.
