03.31.24 Notice of Incident. Acute Placement (1). CLR

Client was transferred to Methodist Acute after making homicidal and suicidal threats with plans and intent to use any object she could find. Client will be returning to the facility once discharged from acute.

03.27.24 Notice of Incident. Medical. CLR

Client complained of stomach pain due to constipation. A leave order was placed by the provider and the guardian transported the client to Arkansas Children’s Hospital for evaluation. Client was admitted into ACH for further evaluation. On 4/1/24, facility reported the resident was diagnosed with fecal impaction.

03.26.24 Police Report. Disturbance. CLR

Officers responded to an unknown trouble at the listed location in reference to a 911 hang up. Upon arrival, officers made contact with Scott who stated someone called on accident trying to use the phone improperly. Scott advised officers were not needed.
