Staff stated they have 4 girls that ran into the woods about 5 minutes prior. After taking descriptions, Staff informed Officers that the girls were coming back out of the woods.

Upon arrival, Juvenile stated that her area had bed bugs and that a staff member pushed her down to the floor causing a bump on her head. Staff stated they were unaware of any incident and that Juvenile has stated several times if she calls the police they would take her to a different place to stay. Officer called the Child Abuse Hotline, it was not accepted.

Staff stated they did not need an officer to come out, she just needed to make a report that they had a fire on 5/15/21 at one of their houses. Staff advised the fire department came and put the fire out.

Juvenile Protection Officer from Nevada advised that she wanted to have an officer go and talk to Juvenile because she had made some allegations against staff for sexual assault; No other info.

Staff stated that on 5/6/21, a patient reported that a staff member had hit him in the head and held his head against the bed frame afterwards. Staff stated she called the abuse hotline on 5/6/21 and they wouldn’t accept the report. Staff stated she talked with the staff member who was accused, and the staff member stated it didn’t happen but that there was an altercation between him (the patient) and other patients. Staff stated she had put the accused staff member on leave until she could finish her investigation. Staff stated there were no marks or bruises on the patient and there is no video footage or witness to the allegation.

Escape from Youth Service Program, Second Degree Escape
Officer received a call on 5/22/21 at approximately 1524 hours that 2 Juveniles had left the facility. On 5/23/21 at approximately 1815 hours, Officer was advised that the 2 Juveniles had been located by Jefferson County Sheriff’s Office; Jefferson County stated they would hold the Juveniles until they could be transported back to facility. Facility advised someone would be on the way to get both Juveniles.
